Review of children’s records revealed that some files were missing currently physicals, enrollment forms, first aid, and emergency medical authorization. In addition, some progress reports and transportation plans, were not dated.
The program did not have a current Staff schedule posted on the day of the visit.

On the day of the visit educators did not have an attendance sheet on the playground. They reported that it was inside in the classroom.
Licensor observed one child alone with a person working with on researcher with no supervision from Program educators.
The program did not have an adequate depth of impact absorbing material under the swings on the day of the visit.

Review of a staff record for an Educaator hired in August of 2021 did not include the following: 1) Staff form 2) Evidence of BRC suitability. 3) Evidence of current physical and immunity to MMR or 2 doses of MMR vaccine. 4) Evidence of First aid certification 5) Evidence that the Educator completed an orientation including EEC essential trainings.
A review of children's records revealed that some files are missing the date of admission, children’s transportation plans, and evidence of Lead test as required for preschool children by DPH.
On the day of the visit the emergency evacuation procedures were not posted near the exit doors in one classroom.
