A random review of children's files showed that not all required yearly updates had been completed. Some files were missing medical records, lead test results, progress reports. Additionally, some IHCPs and medication consent forms were missing required signatures, and some forms were expired.
A random review of staff records revealed that documentation of MMR immunization was missing from one staff file.
There was no documentation available at the time of the visit indicating that monthly evacuation drills had been conducted.
