The following were observed: -A cord over six inches was accessible on the playground. -An access panel on the I/T playground was open exposing insulation and utility equipment. -There was chipping paint on the gate. -Staff purses were accessible in the downstairs PS room. -One shelf was tippable in the upstairs room beside the sink.
