At the time of the visit, the thermometer within the programs refrigerator was reading at 0 degrees. The licensee must provide refrigeration and storage for food at no less than 32F, nor more than 40F for all food requiring refrigeration. Meals and snacks provided by parents must be stored safely. Thermometers verifying temperatures must be maintained in all refrigerators.

At the time of the visit, in a review of four educators files, the documentation was not fully complete. Three educators were missing documentation of bi-monthly observations. Educator B had an expired physical. The program must main a personnel record for each staff member as outlined by regulations.

At the time of the visit, in a review of the staff meeting log, the documentation was not fully complete. Documentation of the October staff meeting was missing. Documentation of the times of the November 2025 - March 2026 meetings were missing. The licensee must develop and follow a written plan and must document regular staff meetings of at least two hours per month to consult with educators on program issues; program planning; policies and procedures; parent communication; implementation of the regulations; children's behaviors and meeting the individual needs of children.
At the time of the visit, in a review of two IHCPs and the corresponding prescribed medications, Student A was prescribed a liquid medication, and did not have a measuring tool on site that matched the dose prescribed. The program must maintain the proper medication/measuring tools to ensure they can immediately provide the proper dose of medication when needed.
At the time of the visit, in a review of two students IHCPs the documentation was not fully complete. Both students were missing parent signatures on their EEC IHCPs. Student B was diagnosed with two conditions. One of their IHCPs was expired. The other IHCP was missing documentation of their diagnosed condition. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

At the time of the visit, in a review of four students files, the documentation was not fully complete. Two students did not have completed Transportation Plans. Children's records must have all required applicable documentation as outlined by the childrens record checklist.
At the time of the visit, in a review of three staff files, the documentation was not fully complete. Two educators were missing documentation of program orientation. Educators did not have evidence of bi-monthly staff observations. The program must maintain a personnel record for each staff member as outlined by the staff record checklist.
At the time of the visit, in a review of the programs emergency drill log, the documentation was not fully complete. Documentation of the October 2025 drill was missing. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
At the time of the visit, in a review of two students IHCPs and the corresponding medication consent documentation, the medication consent documentation was not fully complete. Both students did not have their medication consent documentation signed or dated by their parent. Student A was prescribed two medications, both of their medication consent documents were missing name/phone number of prescribing practitioner. One was missing the dates for the medication to be given, the potential side effects and directions for storage. Student B was diagnosed with two health conditions. Both of Student B's medication consent documents were missing the directions for storage. One was missing documentation on if they had taken the medication before, the dates for the medication to be provided, the times the medication was to be provided and the reason for the medication. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required, including proper administration of medication.
At the time of the visit, it was identified that the refrigerator on site was not yet plugged in and did not have a thermometer. After the visit the Area Manager identified through email that a thermometer was ordered. After the visit, the Area Manager identified through email that EEC Orientation and Champions orientation were printed and placed in files. At the time of the visit it was identified that two educators verified each other's work experience, with their titles listed at "teacher". The work experience form states that "employers" can attest to previous work experience. Please have the two educators contact their past employer's for work experience documentation.
At the time of the visit, in a review of the first aid kit and extra supplies on site, the program did not have tweezers on site. The licensee must maintain adequate first aid supplies, including, but not limited to: adhesive tape, band aids, gauze pads, gauze roller bandage, disposable non-latex gloves, instant cold pack, scissors, tweezers, thermometer, and CPR mouth guard.

Within the document library the program has uploaded a projected budget but has not uploaded financial documentation. The licensee must have and submit a projected one-year operating budget that estimates income and expenses. In addition, a program that has not previously operated must document and submit proof of financial capability to carry out its program for at least a three month period.
At the time of the visit, the cleaning supplies intended for use were not on site. The disinfectant solution used to disinfect child care items, equipment and surfaces must be either a bleach solution prepared by the licensee in accordance with EEC guidelines or a commercially prepared disinfectant that has been registered as a sanitizing solution by the Environmental Protection Agency (EPA). Registration by the EPA will be indicated on the product label. Commercially prepared solutions must be used in accordance with manufacturer's directions.
At the time of the visit, the cleaning supplies intended for use were not on site. All disinfectant solutions must be stored in accordance with manufacturer's instructions and in a secure place out of the reach of children.
