One medication was reviewed. The program did not have a medication consent, signed by the parent giving authorization to the program to administer medication.

The following were noted as tippable: Dress up clothing rack, shelf, tall doll house. A cord to a round sound machine was over six inches and was accessible to children.
Climbing equipment on the playground did not have a 6 foot use zone. (slide composite structure)

The following were observed on the playground: -chipping paint on the white trim between the building and fence, and on the trim above the brick step. -On more than one side of both composite structures, the use zone was less than 6 feet.
Periodic review of current employees/ volunteers
The infant teacher reported that she microwaved water and placed infant bottles in the cup of water to warm. Programs caring for infants must not use a crock pot, (slow cooker), bottle-warming appliance or microwave oven to warm infant bottles. An evaluation of one staff did not contain a date. Evaluations must be done annually. Without a date, it is not possible to determine when that evaluation was done.

Staff meeting documentation did not include the specific date of the meeting or the length of the meeting.
The program did not have evacuation procedures at each exit and did not have a specific plan that addresses the needs of all children in care, including infants, toddlers and any children (including but not limited to those with disabilities) who may need additional assistance during an evacuation, specifically in the infant room.
One pacifier was observed that did not have a child's name on it.
