There is a child in the program who is prescribed [REDACTED], which is considered a lifesaving medication. This information is not currently posted. Licensor discussed with Educator the need for a list to be posted of all emergency or lifesaving medications, including but not limited to [REDACTED]medications that specifies to which child they belong. Licensor also discussed that this information needs to have a cover sheet in order to protect the child’s confidentiality.
Licensor randomly reviewed 3 of the children's records. Of these, 1 had an out of date physical and immunization record, and the other 2 both had out of date immunization records. Please review all of the children's records and ensure they are all complete and up to date.
Licensor randomly reviewed 3 of the children's progress reports. Of these, 2 were complete and up to date, but 1 was out of date.
Licensor inspected all of the indoor, licensed space during the progress visit, as well as the outdoor playspace. The only hazard that was observed was in the diningroom, there was an outlet that was not covered. The gate also was not in place, so the children had access to the kitchen.
Licensor randomly reviewed 3 of the children's records and observed the following: **#1** - physical is out of date **#2** - no signature on pg.6 of the enrollment packet under Basic Transport, out of date physical **#3** - no signature pg. 6 under Basic transport, out of date physical. Please review all of the children's records and ensure they are all complete and up to date.

Educator has an emergency preparedness plan. However, he needs to add a meeting place if the neighborhood needs to be evacuated.
Educator has a record for the Certified Assistant who primarily runs the program. However, the file was missing a medical form and there were no training certificates.
Licensor inspected all of the licensed, indoor space during the visit, as well as the outdoor playspace. The only hazard that was observed was an outlet that was not covered in the dining room. Also, because the kitchen is not licensed, there needs to be a gat in place to make that room inaccessible. The gate was not being used during the visit.
