Health and safety inspection of classrooms found the following: educators' bags within reach of children, a backpack containing emergency medication on a shelf within reach of children, dirty towels on floor in PreK bathroom, nap mats thrown on floor in front of toilets in PreK bathroom, rugs with curled up edges creating tripping hazard and some stained/dirty area rugs.

Administration did not ensure a new educator had an approved BRC prior to working in the classroom. An educator working in the program had a BRC still in process and had not been deemed provisionally suitable or suitable.
Review of medication consents found 3 medications with no medication authorization on file.
An educator in a classroom was unsure why a child had multiple bottles of [REDACTED] in the classroom.
Children in a toddler room did not wash hands prior to playing in sensory table with water and shaving cream.
Review of IHCPs found the following: One child's IHCP states [REDACTED] as part of their emergency plan and there is none on-site, a child's IHCP states [REDACTED] but they have [REDACTED] a child's IHCP expired 8/16/2024 and one child's IHCP states to give [REDACTED] "per instructions and may give [REDATCED[ which does not clearly state when to give either medication.
An open bag of Goldfish was found in a classroom drawer not stored properly.

Random review of 8 children's files found: 2 files with no current lead screening and 1 with an expired consent to receive EI services at the program.
Random review of 6 staff files found the following: 1 file did not contain work experience form and is being used as a teacher and their physical documentation did not contain an actual date of their physical. One file did not contain a current physical or evidence [REDACTED], and one file only had evidence of the Essentials safe sleep training and not the completed Essentials trainings.

Review of IHCPs found the following: There was no IHCP on file for 2 children with chronic medical conditions, 1 IHCP was found expired in 6/2022 and another child's IHCP stated [REDACTED]but there was no stated dose, and the program did not have it on site.

During health and safety inspection the following was found: staff member's purses found within reach of children, a dishrack was visibly dirty, some area rugs curling up on the corners posing a potential tripping hazard, vinyl baseboard in preschool bathroom peeling off of wall, torn nap mats and a red couch has holes on the arms.
The program did not have current IHCP's on file for all children with a chronic health condition. One IHCP was not clear on when to give each specific medicine.
Although not observed most staff described cleaning/disinfecting tables and diaper changing mats with only a one step process. Some using only soap and water and others using only a disinfectant.
2 children's [REDACTED] were found without a prescription label. Some classrooms had medication stored in bags accessible to children.

Review of 3 children's files found 1 file with no [REDACTED] and no progress report at an interval needed for a child with documented [REDACTED].
Review of 3 staff files found the following: 2 files had no current physical, 2 showed no evidence of immunity to [REDACTED], 2 had no current PQ registry form, 1 had no proof of Strong Start essentials training, 1 had documentation of only 1 verbal reference and 1 had no documenation of program orientation.
[REDACTED] has not ensured all staff have either a "Provisional" or "Suitable" background record check hiring status prior to the employee starting work. During the visit Licensor instructed [REDACTED] to send home 4 staff that had an incomplete BRC.
A staff person was observed walking [REDACTED] children to the front office area to get a child's [REDACTED]. This staff person is an [REDACTED] and not qualified to be alone with children.

Furniture and equipment was found tippable posing potential hazards to children. A toilet was found with no toilet seat and no toilet paper.
