Children’s records were reviewed for the five enrolled children presents in the program. Two children require updated medical information, four children need their annual forms updated in their enrollment packets, and one child requires updated lead screening information.
One child with a documented allergy and prescribed life-saving medication was not listed in the Allergies and Medications Form and posted as required.

Emergency cards were missing for all enrolled children. It appears the cards had gotten wet, erasing the information and making it unreadable.
First Aid Kit
The following hazards were observed and accessible to the children: Kitchen: - Although there were locks on the kitchen drawers, one was not working, leaving a toolbox containing hazardous materials including but not limited to a hammer and a variety of tools accessible to the children. - Two uncovered outlets were observed. Living room: There were a variety of cables or equipment wires on the floor accessible to the children near the TV stand. Bathroom: - Sink cabinet doors were not secure. Inside there were cleaning products accessible to the children. - On the floor there was a container of Lyson wipes accessible to the children. Entrance room: A TV cable was not secured to the wall.

In the basement playroom there was not enough developmental appropriate toys accessible for the children in care. Additionally the small white shelf with a puzzle and a bead toy was dirty
