Individual Health Care Plan (IHCP)
At the time of the visit five child files were reviewed and the following was found: one child's file needed the annual updates completed (developmental history, emergency consent, transportation plan) and one child's file needed a current physical and parent signature on transportation plan form.
At the time of the visit the infant classrooms had not signed out all of their children that had already left for the day.
There was noticeable chipping paint in need of repair in the following classrooms: Toddler A, Preschool 1B and Preschool 3.
Monthly evacuation drills were not logged for August 2024 or January 2025. Drills are to be conducted and logged on a monthly basis.
One child listed on the emergency medical condition list did not have any documentation to support their condition.

There were a number of places throughout the center that had some peeling paint in need of repair.
At the time of the visit cleaning supplies were accessible to children in the infant classroom. There were also staff's personal belongings accessible to children in a preschool classroom. Both of these situations were due to broken cabinet latches.

Health and Safety
In the Preschool 1 Classroom, a box of vinyl gloves was stored on a shelf accessible to the children. The gloves were removed during the visit. Educators must ensure that all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags, and purses are locked or inaccessible to children.

Infection Control
During the visit, Four staff files were reviewed. One file did not include an interview date. One file did not include verification of two references. Three files did not include evidence of Program Orientation. One file did not include documentation of PQ Registration. Three files did not include documentation of EEC Essential trainings.
In the Twaddler A and Preppers classrooms, mops and brooms were stored on the floor in the children's bathrooms. A toilet brush was stored in the Twaddler A bathroom and accessible to the children.
A staff schedule was not posted in the program as required. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents, and visitors. This was previously cited on 11/13/2021.
The program did not maintain a Staff Record Checklist that reflected all staff who were in ratio in the program. One educator working in the Twaddler A classroom was not listed on the checklist. This was previously cited on 11/13/2021.

One educator did not have current first aid certification. All educators must obtain within six months of employment, and must maintain thereafter current certification of training in basic first aid appropriate to the population served.
Two children's sippy cups were not labeled with the children's names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes, and sleeping materials, must be labeled with the name of the child for whom they are intended.
The program's licensed capacity requires the Program Director to be 100% non-teaching administrative time. A review of the staff schedule provided during the visit shows the Program Director working in the Preschool 1 classroom 4 days per week. This was previously cited on 11/13/2021
Bi-monthly observations of educators while working with children at least every two months, were not regularly conducted by a staff with lead teacher, site coordinator, or higher qualifications. This was previously cited on 11/13/2021.
A child's bowl and spoon were observed on the counter next to the handwashing sink in the Preschool 1 classroom. Three children's thermoses and one sippy cup were observed on the counter next to the handwashing sink in the Preppers classroom.
