The records for the seven enrolled children were reviewed and found to be incomplete. Missing information was noted under the Basic Permissions section — specifically for General Permission (Basic Transport) and Permission to Transport to a Medical Facility and Receive Emergency Medical Treatment. In addition, three children need updated annual forms, and one child’s medical record needs to be updated.

The following safety hazards were observed and were accessible to children in the outdoor play space: During an inspection of the educator's backyard, licensor observed that the fence separating the educator’s yard from the neighbor’s property had fallen, exposing several safety hazards accessible to children. These hazards included an above-ground pool with an unbarricaded deck allowing open access, gallons of gasoline, a ladder, garden tools, metal scrap materials, empty buckets filled with dirty water, and a water hose lying on the ground. In the educator’s backyard: A water hose on the ground creating a tripping hazard, and a garden hoe was also observed.
A hammock was observed near the fence in the outdoor space without an appropriate impact-absorbing surface underneath and without the required six-foot clearance around it.
In the bathroom, wipes were observed to be accessible to the children.
This progress visit inspection recorded no violations or advisories.

Provider did not have enrollment forms completed for any of the children enrolled.
Provider did not have an appropriate emergency alternate location in her emergency plan.
The first aid kit was missing an instant cold pack, tape, the children’s emergency cards, tweezers and thermometer. The tweezers and thermometer were added to the first aid kit during the visit.
Provider has only completed 4.5 hours of ongoing professional development. Provider is required to complete 10 hours annually.
Provider has not been documenting daily attendance since she has been licensed.
The bathroom cabinet under the sink had a lock but was unlocked allowing access to hazards to include rubbing alcohol, aerosol air freshener, chloride solution and a bottle of hand sanitizer.
A blender blade was accessible on top of the kitchen counter. There were two long charging wires were hanging and accessible to the children. In a cabinet which was unlocked were plastic bags. A toy bin and a toy kitchen in the living room were not secured causing a tipping hazard.
