Safe Sleep
The following hazards were observed and accessible to the children: BATHROOM: The vanity door was not secured with hazardous products inside, including but not limited to spray disinfectant, hair products such as shampoo and conditioner, hair dryer, oral rinse and air freshener.
During the visit, the Licensor did not observe the two infants in their nap time. However, upon inspection of the playpens, it was noted that one of the two playpens had an additional soft mattress on top of the original one provided by the manufacturer, and the other playpen had a boppy pillow inside. The educator stated that the (REDACTED) sleeps in the playpen with the soft mattress, while the (REDACTED) does not sleep in the other playpen. The educator mentioned that she holds the (REDACTED) until fall sleeps in her lap and sometimes the Educator lets the infant sleep on the couch while she sits on the couch (REDACTED).
Provider did not have the EEC handbook.

Provider’s had not documented any evacuation drills since the month of September as well as August not being completed prior. Provider is also not documenting the time of the drill or alternating exits.
In the bathroom cabinet under the sink there were gloves, cleaning supplies and spray accessible to the children. In the kitchen there were knives, scissors and medication accessible in the kitchen drawers.
Provider did not have the EEC handbook which includes the emergency preparedness.
The front door exit was being obstructed by the Christmas tree.

Outdoor Playspace - Hazards
The first aid kit was missing scissors, medical tape, tweezers, an instant cold pack and the children’s emergency cards. The provider added the medical tape and scissors during the visit.
There was one electrical outlet uncovered in the playroom and one on the kitchen island. There was a snowman decoration in the window of the playroom with an electrical cord hanging from it. A radio with the electrical cord was also in the playroom accessible to the children.
There was water over 3 inches deep in an open sandbox.
Educator had two climbing structures (2 slides) that did not meet the required fall zones of 6 feet all around.

Exit Safety
The Educator is not documenting time in/time out daily, as required. This is not the first time the Educator is cited for not having accurate attendance.
Four child care booklets need the annual update form completed by the parent. Two medicals are over a year old and one child has only vaccinations on file.
The last documented fire drill was in January. Fire drills must be documented monthly.
The Licensor observed a collapsed pool in the outdoor play area that is accumulating water. The Licensor observed 5 latches that are not working exposing hazards such as knives and other hazards in the kitchen. The Licensor observed hazards under the bathroom sink such as make up, sprays, shampoos and lotions. The Educator placed a latch while the Licensor was present but there was no latch until the Licensor explained that these hazards are dangerous to the children.
The front door was blocked by a small trampoline. The trampoline can’t be used with children. Exits needs to be accessible.
Nap items are not labeled. This is not the first time the Educator is cited for the same violation.
