The educator does not have the required 6-foot clearance around the three slides in her backyard. According to EEC requirements, the fall zone for each piece of equipment must extend a minimum of 6 feet in all directions from its perimeter.

The licensor observed the (REDACTED) pack and play to have a blanket in place of the tight fitted sheet that is required, additionally there were 2 pack and play mattress in the pack and play. The educator stated she felt the surface to be to hard for the(REDACTED) so she was trying to make it softer. The (REDACTED) infant (REDACTED) and since (REDACTED) the educator’s has been sleeping (REDACTED) an (REDACTED) seat. The educator does not have a doctor note stating he can sleep in something other than the pack and play
