General: Based on review of documentation, the surveyor found a staff’s (DOH: 02/23/22) file did not contain documentation of background checks submitted through the Kentucky National Background Check Service. Based on review of the Kentucky National Background Check Service, the staff member does not have a completed background check; therefore, the staff person was hired prior to clearance for employment. The staff’s file contained a completed Child Abuse/Neglect Background Check dated 02/28/2022 and a completed Criminal Records Background Check dated 01/12/2022. Staff-in-charge stated a background check was submitted for the staff through the Kentucky National Background Check Service; however, staff-in-charge was not sure if the staff had completed their fingerprints. A tall wood framed mirror placed behind a cot that was in the floor in Wee Care Classroom 1. The surveyor did not observe a child in the cot. 3. A red dolly and metal platform dolly with wheels located in a hallway near the restrooms. 4. A lunch room table folded and standing upright next to a wall in the hallway by the restrooms. 5. Four (4) boxes with orange cables coming out of them placed in the floor of the hallway by the restrooms. During interview, staff-in-charge stated that Wee Care Classroom 2 uses the restrooms in the hallway. 6. A large box and white standing fan placed by the door that leads to Wee Care Classroom 2. 7. Eight (8) children and one (1) staff person sitting in a gazebo located outside the fenced in area of the playground. Staff-in-charge stated that the eight (8) children and staff person were waiting to go onto the playground as another classroom was currently on the playground. The surveyor did not observe children's files to contain written permission specific to allowing the children to go to the gazebo which was located outside the fenced in area of the playground.
