General: Based on review of documentation of KARES, a staff member (DOH: 08/08/23) was not listed on the KARES report; therefore, the surveyor could not determine if she was eligible to work at the child-care center. General: Based on observation, the surveyor discovered that a staff member (DOH: 08/08/23) was working with another staff member in the infant classroom with ten (10) children aged from seven-week-old to eleven-month-old. During review of documentation, the surveyor learned that the staff member (DOH: 08/08/23) was not listed on the KARES report; therefore, the staff member was working with children before having an eligible status on the KARES report and meeting the requirements to be qualified. General: Based on observation, the surveyor arrived to the child-care center at 12:59 pm to observe three (3) children lying down on cots in the Toddler 2 Classroom. The surveyor observed the three (3) cots to be touching; therefore, the three (3) cots were not at least twelve (12) inches apart. When the surveyor entered the Toddler 2 Classroom at 1:26 pm, the surveyor observed the three (3) cots had been moved and were at least twelve (12) inches apart from each other. General: Based on observation, the surveyor found the following: Infant Classroom: 1. The surveyor observed a diaper changing table inside the Infant Classroom. The Infant Classroom does not contain a hand-washing sink. The surveyor reviewed the diagram that was signed by the staff-in-charge on 02/20/24 to find that a diaper changing table was not originally placed in the Infant Classroom. During interview with staff-in-charge, she stated a diaper changing table was added to the Infant Classroom after the child-care center opened to make it easier on staff when changing children’s clothes. Staff-in-charge reported staff use the diaper changing pad in the restroom to change children’s diapers. Toddler 1: 1. The surveyor observed a diaper changing table insid the Toddler 1 Classroom. The Toddler 1 Classroom does not contain a hand-washing sink. The surveyor reviewed the diagram that was signed by the staff-in-charge on 02/20/24 to find that a diaper changing table was not originally placed in the Toddler 1 Classroom. During interview with staff-in- charge, she stated that a diaper changing table was added to the Toddler 1 Classroom after the child-care center opened to make it easier on staff when changing children’s clothes. General: Based on observation and interview, a cold pack was not presented at the time of the inspection. During interview the staff in charge acknowledged the findings
General: Based on observation, there was a hole in the wall above the urinal located in the boys bathroom.
General: Based on interview and review of documentation, during the inspection, the following was found: 1. The statement of deficiency issued for the current licensure year was not posted. 2. The plan of correction submitted to the cabinet for the current licensure year was not posted.
General: Based on observation, the refrigerator located in the infant room did not contain a thermometer or other temperature measuring device; therefore, it could not be verified that the temperature was forty (40) degrees Fahrenheit or below.
General: Based on interview and review of documentation, the personnel file for two (2) staff members (DOH: 03/20/22, and 08/08/23) did not contain an evaluation for the year 2024. Upon request, the documentation was not presented.

General: Based on observation, during the inspection, the following was found: One-Year-Old Room 1) A child was diapered and permitted to return to participate in activities without washing his/her hands. Pre-K Left 1) Children were provided hand sanitizer to use before breakfast; therefore, the children did not wash their hands with liquid soap and warm running water. Bathroom off the office 1) A child washed his/her hands then started touching the toilet seat, rubbing his/her hands all around the seat area, without being directed to wash his/her hands again by the staff member.
General: Observation of the Pre-K Right room revealed a person identified as a third-party contract employee working with children at the time of the inspection. Upon request, a completed background check nor a third-party contract was presented for the aforementioned person. There was a qualified staff member in the classroom during the inspection.
General: Based on observation, the Kentucky Applicant Registry and Employment Screening (KARES) roster did not list five (5) of the current staff members. Interview with the staff in charge revealed the staff members are on the roster for another child care center under the same ownership. Background checks for the staff members were verified. General: Based on observation, during the inspection, the following was found: Infant Room 1) There were two outlets that were not covered. The outlets were located above cribs. The staff member did not acknowledge the incident until the surveyor informed them of the observation.
General: Based on interview and review of documentation, the personnel files for staff members (DOH: 07/01/25, 08/05/25, and 07/29/25) did not contain a high school diploma, GED or Commonwealth Child Care Credential at the time of the inspection. Upon request, the documentation was not presented.
General: Based on interview and review of documentation, there was no staff member certified in cardiopulmonary resuscitation (CPR) and first aid. Upon request, the documentation was not presented.
General: Based on interview and review of documentation, during the inspection, the following was found: 1) There was no daily planned program posted in the infant classroom. 2) There was no daily planned program posted in the one-year-old classroom. 3) There was no daily planned program posted in the and two-year-old classroom.
General: Based on observation, there were three (3) purses sitting on the floor in the office. Children used the office as a walkway to go to the bathroom making them accessible to children. During interview a staff member confirmed that the three purses belonged to staff members.
General: Based on observation, during the inspection, the following was found: Pre-K Right 1) There was a section of baseboard that was detached from the wall located under the window.
General: Based on observation, the lights in the kitchen were not shielded. During interview with the staff in charge it was not verified that the bulbs were shatter proof.
General: Based on observation, during the inspection, the following was found: One-Year-Old Room 1) The diaper changing area was not adjacent to a sink at the time of the inspection. A diaper change was observed in the one-year-old room. Boys Bathroom 1) The water temperature in the sink registered seventy-eight degrees Fahrenheit.
General: Based on observation, during the inspection, the following was found: 1) A staff member did not wash his/her hands before preparing a bottle. 2) A staff member used gloves while changing diapers in the one-year-old room; he/she did change the gloves before diapering the next child. The staff member did not wash his/her hands between diaper changes.
General: Based on observation, the changing pad located in the one-year-old classroom contained tears exposing the inside absorbent material.

General: Based on observation, during the inspection, the following was found: Pre-K Left 1) There was a wooden cabinet with two (2) screws that were protruding, that were accessible to children. Toddler 2 1) Two rugs (I am feeling rug and the square rug) were soiled with debris. Pre-K Right 1) A rug that was rolled up causing a tripping hazard.
General: Based on observation, during the inspection, the following was found: Infant Room There were two (2) bottles that were not covered located on a shelf with a white liquid in them. General: Based on observation, there were two (2) infants reclining in individual bouncy seats consuming a bottle.
General: Based on observation, there were cans of infant formula in children’s cubbies. During interview with a staff member, it was verified that the parents send in infant formula and bottles are prepared by the child care center staff.
General: Based on interview and review of documentation, the record for a child (DOE: 08/06/25) did not contain an immunization certificate. Upon request, the immunization certificate was not presented for review.
General: Based on interview and review of documentation, during the inspection, the following was found: 1) The record for a child (DOE: 08/07/25) did not contain the contact information for the preferred hospital and physician. 2) The record for a child (DOE: 04/08/24) did not contain permission for third-party professional services while in care at the child care center.
General: Based on interview and review of documentation, during the inspection, the following was found: 1. There were not job descriptions for each position. The only job description presented was for the teacher assistant role. 2. There was not an organizational chart. 3. There was not a chain of command.
General: Based on interview and review of documentation, documentation of fire, earthquake, tornado and shelter-in-place drills were not presented. Interview with the staff in charge revealed the drills had been conducted.

General: Based on review of documentation and ECE-TRIS, the surveyor found that the following staff members (DOH: 08/08/23 and 01/29/24) did not complete the required cabinet approved training in first aid and cardiopulmonary resuscitation (CPR).
General: Based on review of documentation, the surveyor found the following: 1. A child's (DOE: 03/18/24) file contained an immunization certificate that was no longer current as of 06/01/24. 2. A child’s (DOE: 04/05/24) file did not contain an immunization certificate. 3. A child's (DOE: 04/22/24) file contained an immunization certificate that was no longer current as of 06/06/24. 4. A child's (DOE: 04/29/24) file contained an immunization certificate that was no longer current as of 05/09/24. 5. A child's (DOE: 05/01/24) file contained an immunization summary report that did not provide documentation of when the child's immunization certificate expired. 6. A child's (DOE: 06/03/24) file contained an immunization certificate that had a box checked "immunization not-up-to-date" with the date valid until 06/17/24.
General: Based on observation, the surveyor found the following: Preschool 4 & 5 A Classroom: 1. There was a large rolled up rug placed on the floor in front of the children's cubby area that created a potential tripping hazard for the children in the classroom. 2. The top shelf of the cubby area contained two (2) pencil sharpeners that had cords hanging from the shelf into the floor. The cords were observed to not be secured. Preschool 4 & 5 B Classroom: 1. There were five (5) cots standing upright in the back corner of the classroom. The cots were observed to not be secured and could easily be moved. Preschool 3 Classroom: 1. There was a fish tank placed on top of a shelf that had several cords coming out from around the fish tank and hanging downward toward the floor. The cords were observed to not be secured and accessible to the children. 2. There was one (1) cot standing upright in the back corner of the classroom. The cot was observed to not be secured and could easily be moved. Therefore, the health, safety and comfort of the children was compromised. General: Based on review of documentation, the surveyor found a staff member's (DOH: 06/04/24) file that contained documentation from KARES that the staff member was eligible for hire on 05/29/24. On the day of the surveyor’s visit, the surveyor printed and reviewed the child-care center’s KARES report. The staff member was not listed on the the child-care center’s roster; therefore, the child-care center failed to update the staff member’s employment status. General: Based on review of documentation, the surveyor found two (2) staff member's (DOH: 03/30/22 and 02/20/24) files did not contain documentation of a current TB skin test or a statement from a health professional stating that the adult was free from active tuberculosis. During interview with staff-in-charge, she stated that she did not have current TB skin tests for either staff members.
