General: Based on observation, there were a total of three (3) staff present inside the child care center on 7/11/17. The following was observed during this visit: 1) One (1) staff member (DOH: 11/18/11) provided supervision to the Toddler and the School-Age Classrooms from 10:58am-10:59am while another staff member (DOH: 2/1/13) used the restroom. During this time, staff member with DOH: 11/18/11 stood inside of the School-Age Classroom and was supervising the Toddler Classroom by looking over the shelf wall that separates the rooms. The minimum staff-to-child ratio was not met during this time, i.e., one (1) staff was responsible for supervising nineteen (19) children, with the youngest child being three (3) years of age. 2) One (1) staff member (DOH: 2/1/13) provided supervision to the Toddler and the Baby Classrooms from 11:42am-11:49am while another staff member (DOH: 4/20/12) changed children's diapers inside of the bathroom located off of the School-Age Classroom.
