General: Based on interview, a three-year-old child had been feeling poorly and fallen asleep on a classroom couch. The staff members in the classroom prepared the class to go outside to the playground at approximately 10:05 am and failed to complete face-to-name protocols, per their statements, resulting in the child being left inside, alone, sleeping on the couch. A staff member who had been on break met the class outside on the playground at approximately 10:10 am and identified the child as missing from the group and immediately contacted the staff in charge, who entered the classroom, collected the unharmed and unalarmed child, and returned her to her class on the playground. The two (2) staff members with the group that had left the child in the classroom were placed on administrative leave on the date of the incident, 10/03/25, and formally terminated from their positions on 10/07/25. General: Based on review of documentation and the Training Records Information System (TRIS), the following was found: 1. One staff person (hire date: 8/27/84) did not have evidence of completing the required ECE 202 training class (Requirements for Reopening Child Care in Kentucky). 2. The facility failed to submit a background check for two staff (hire dates: 8/24/20 and 8/31/20). Information for both staff had been entered in KARES and they were listed as "in process". Both staff also had a completed criminal records check (CRC) on file; however, there was no evidence or results on file of a completed child abuse/neglect check (CAN). The staff person in charge stated that these staff do not work alone with the children.

General: Based on Observation, and interview, the facility failed to meet this regulatory requirement. A tour of the facility during the investigation found two (2) classrooms (Room #105 and Room #107) that had ceilings in disrepair. Room #105 had eleven (11) missing ceiling tiles throughout the room. Room #107 had one (1) missing ceiling tile, two (2) ceiling tiles that contained dark stains and one (1) ceiling tile that fell in the floor from being saturated with water. The staff person in charge stated that these two (2) classrooms are not currently being used by children or staff due to low enrollment and indicated that ceiling tiles are being removed from the ceiling in these two (2) rooms to replace damaged ceiling tiles in other classrooms. The staff person in charge stated that the facility maintenance staff are aware of where the leaks are and indicated that they watch them close on a daily basis right now while snow is melting on the roof.
