General: Based on observation and interview, the surveyor arrived at the center on 12/19/25 at 9:25 a.m. The surveyor rang the doorbell twice, noting that the lights appeared to be off in classrooms visible from the sidewalk. It was later explained to the surveyor by an administrator that the center closes for an in-service day monthly and does not report those closures to DRCC. The surveyor explained the process to report any and all closures going forward and the administrator stated she would submit a list of future closures for in- service.

Before departing, a child in care fell in the grass while playing soccer and complained his arm was hurting. Upon return to the center, the child was taken to the local emergency room by his mother for evaluation. It was found he broke his clavicle bone.

General: Based on observation, review of documentation, and interview, this regulatory requirement was not met. On 5/4/19 it was reported to the DRCC office that a staff person (D.O.H. 3/21/19) lifted a three (3) year old child and put them down in an unprofessional manner and the staff person's employment was terminated as a result. An interview with staff during the investigation revealed that the three (3) year old child frequently exhibits aggressive behavior towards staff and other children. On 5/3/19 the child pushed another child out of a chair in the book area that she wanted to sit in. At this time the staff person picked the child up by her outer arms and threw her down on the floor. The staff person was visibly frustrated with the child at the time. The staff person's termination paperwork stated that on 5/3/19 there was an incident in the classroom that she was assigned to that involved inappropriate behavior by her towards a child in the classroom. The behavior was unacceptable employment behavior and a lapse of professional judgement. Staff stated during interview that the child did not have any visible injuries as a result of the incident; however, indicated that the child cried as a result of the incident and appeared scared.

General: Based on Observation, DRCC surveyor arrived inside the facility lobby at 9:52am. The doors to the daycare were locked and a security pad and a call button were next to the doors. There was a front desk person present and the surveyor asked her to let the daycare know a visitor was there. She radioed back to the daycare three (3) times to let them know someone was at the front door. After a couple of minutes the surveyor rang the call button but no one responded. After five (5) minutes someone finally came to the door to let the surveyor in. The director stated that the front desk person works for the apartment building and not the daycare. She also stated that the call button was not working properly.
