General: Based on observation and interview, the child care center failed to maintain the premises in accordance with regulatory requirements. 1) The first toilet located in the Girl’s Bathroom would not flush and had an out of order sign on the toilet at the time of the survey. 2) The second toilet located in the Girl’s Bathroom was not secured to the floor. 3) The third toilet located in the Girl’s Bathroom had a broken toilet tank lid at the time of the survey. 4) The second toilet located in the Boy’s Bathroom would not flush and was not secured to the floor at the time of the survey. 5) The toilet tank lid on the third toilet located in the Boy’s Bathroom did not properly fit the tank at the time of the survey. 6) The toilet tank lid on the toilet located in the bathroom inside the Three and Four-Year-Old Room was completely taped up at the time of the survey. Interviews revealed that the owner turns the water off to the toilets every night and the staff members are responsible for remembering to turn the toilets back on every morning. General: Based on observation, the child care center failed to comply with regulations regarding director requirements. General: Based on review of documentation and interview, the child care center failed to comply with regulations for employee records. Review of presented staff files revealed two (2) staff members with the hire dates of 10/09/2015 and 06/09/2014 did not complete the fifteen (15) hours of cabinet approved early care and education training. Review of the ECE TRIS report verified the training hours were not completed. Interview with the director revealed she was not able to verify the training hours were completed. Further review of documentation revealed the presented Pediatric Abusive Head Trauma Training documentation for three (3) staff members with the hire dates of 11/01/2014, 12/08/2006, and 10/06/2006 were no longer current. General: Based on review of documentation and interview, the child care center failed to comply with regulations regarding posted documentation. Review of posted documentation revealed the Statement of Deficiencies issued by the cabinet and the Plan of Corrections submitted by the center during the current licensure year were not posted in a conspicuous place. Interview with the director revealed the Statement of Deficiencies and Plan of Corrections were in the outer corridor adjacent to the director’s office, which was viewed to not be conspicuous to a parent’s view. General: Based on Observation, the child care center failed to maintain the premises in accordance with regulatory requirements. Observation of the Two and Three year old room revealed there was not enough adequate lighting, 2 ceiling lights were observed to be not working. Further observation revealed there was not adequate light in the Blue Room A, Blue Room B and the Girls bathroom. And the Infant room has a ceiling light out with no protective covering.
General: Based on Observation, the child care center failed to maintain the equipment in accordance with regulatory requirements. Observation of the Infant room revealed there was a blue rocking chair that had stained fabric. General: Based on interview and observation, the child care center failed to comply with regulatory requirements to maintain ratios and group size. Interview with the staff member in charge of the small children's play revealed it contained thirteen (13) children, aged 3 years to 5 years, from 9:40 AM to 10:05 AM. At 10:05 AM, the Director was observed removing two (2) older children from the small children's outdoor play area, which then brought the use of the outdoor play area into compliance. Observation and interview with another staff member in a School Age classroom revealed that she did not arrive at the center until 10:15 AM. Additionally, the Director acknowledged in interview that the center was initially understaffed because a staff member arrived late that day. 45 - Ratio/Group Size Discussed 922 KAR 2:120. Section 2. Child Care Services. (2)(b) The age of the youngest child in the group shall determine the: 1. Staff-to-child ratio; and 2. Maximum group size.
General: Based on observation, the child care center failed to comply with equipment regulations requiring that mats used for napping be kept 12 inches apart. Observation of naptime in the Three's/Four's class room revealed that nine (9) of twelve (12) mats were placed from 3 inches to 7 inches apart.
General: Based on observation, the child care center failed to comply with regulatory requirements requiring outdoor playground be be well-maintained and safe from forseeable hazards. Observation revealed a broken latch on the gate separating the small children's outdoor playground from a larger outdoor playground. Observation upon entering the larger playground revealed a large rusty bolt extending above the concrete play area, representing a potential trip hazard. Observation further revealed the large outdoor playground was bisected by several metal posts with chicken wire to create a sort of fence separating the concrete play area from a fenced grassy play area. The metal posts measured 2 feet, 11 inches tall and some of the ends were noticeably sharp upon touch. The attached chicken wire was bent and varied in height from 1 foot 10 inches to 2 feet 4 inches in height, representing an additional trip hazard for children trying to access the grassy play area or re-entering the concrete play area.
General: Based on Observation, the child care center failed to comply with equipment regulatory requirements. Observation of the playground revealed a red metal car that was rusting in various areas. Continued observation of the aforementioned car revealed that the seat was in disrepair and contained separating duct tape.
General: Based on Observation, and interview the child care center failed to comply with premises regulatory requirements. Observation of girl's bathroom near the lunchroom revealed two (2) toilets with paper over them stating to do not use. Interview with the director revealed the aforementioned two (2) toilets have not been working for approximately one (1) week. Hygienic Practices
General: Based on Observation, review of documentation, and interview the child care center failed to comply with premises regulatory requirements. Observation of an 845 square foot classroom revealed it was not suitable for its intended purpose. Observation of the aforementioned classroom revealed it had various clothing, toys, and household items sitting on tables with price tags on the items. Review of documentation revealed a flyer lying on a table in the aforementioned classroom that stated the facility has a classroom that they are not using due to low enrollment. Continued review of documentation revealed the aforementioned flyer stated that parents can bring in "yard sale items" to be displayed and sold. Interview with the owner revealed the aforementioned classroom has not been used since December 2014. Interview with the director revealed the in-door yard sale display did not do too well and that they were thinking about closing it down.
General: Based on Observation the child care center director failed to assure the health, safety and comfort of a child. Observation of the playground revealed there was an area located near the fence line that contained a concrete border, which dropped down into a dirt area. The concrete appeared jagged and had broken edges. The condition of the playground resulted in an injury to a five-year-old child resulting in scrapes to his knee. Outdoor Play Area
General: Based on Observation and interview with the director, the child care center failed to maintain an outdoor play area in accordance with regulatory requirements. Observation of the playground revealed there was an area located near the fence line that contained a concrete border, which dropped down into a dirt area. The concrete appeared jagged and had broken edges. Continued observation revealed there was a metal stake protruding from the ground located at the edge of the concrete area leading to the grassy area, along with another metal stake near the Rubbermaid cabinet, therefore the outdoor play area was not maintained and in good repair and was not safe from foreseeable hazards. Interview with the director revealed she was aware of the issues.
General: Based on Interview and Observation the child care center failed to comply with regulatory programming requirements. Upon entrance into the Toddler Room at 11:33 a.m. interview with staff revealed that there were five (5) one-year-olds and one (1) two-year-old currently in the classroom. Continued observation revealed that the two-year-old stayed in the Toddler Class with the one-year-olds for the duration of the survey ending at 2:45 p.m.
General: Based on Review of Documentation, the child care center failed to maintain employee records in accordance with regulatory requirements. Record review revealed that the center failed to submit the Criminal Records Check on the date of hire for staff members with the hire dates 3/3/14, 4/22/14 and 6/9/14. Continued review revealed that staff member with the hire date 3/3/14 Criminal Record Check was dated 3/26/14 and staff members with the hire date 4/22/14 and 6/9/14 Criminal Record Checks were requested two (2) days after their date of hire. Continued review revealed that the center failed to submit the Child Abuse and Neglect Check within five days of hire for a staff member with the hire date 3/3/14. Further review revealed that staff member with the hire date 3/3/14 Child Abuse and Neglect Check was dated 3/28/14.
General: Based on Review of Documentation, the child care center failed to maintain employee records in accordance with regulatory. Review of employee records revealed that a staff member with the hire date 4/22/13 tuberculosis test was no longer current after 6/8/14 therefore, the center failed to have a current statement from a health professional that she is free of active tuberculosis.

General: Based on observation, the abandoned building that bordered the outdoor play area that was paved (and served as the walkway to the fenced outdoor playground) was in poor condition and appeared to be falling down; i.e., the siding was buckled, the building was leaning and the wood on the roof was rotting.
General: Based on observation, the fenced outdoor playground was closed and renovation was being conducted. There was some flooding, the mulch had been removed and the fence was in disrepair. The outdoor play equipment was taken apart and stored next to the fence. Interview with the director revealed the renovation had been underway for approximately one week.
General: Based on observation, the following was found: 1.) There were several areas of chipping paint on the walls within the School-Age classroom. 2.) A portion of drywall within the Two-to-Three-year-old bathroom was chalking and not smooth. 3.) A baseboard was not securely fastened to the wall within the One-Year-Old classroom and Three-Year-Old classroom.

General: Observation of the double glass doors within the School-Age room revealed a large gap in the area where the two (2) doors join, allowing daylight to show through between the doors and the cool air could be felt coming through the multiple gaps between the doors. General: Observation of the floor within the Eating Area revealed there were multiple floor tiles missing and the rough concrete floor was exposed. There were stained ceiling tiles in the Two-Year-Old room and the Boy’s Bathroom, indicative of water damage. There were several ceiling tiles that were not secured into the ceiling frame in the Girl’s Bathroom and the Eating Area. There was a hole in the hallway wall. In the School-Age room there was a hole in the wall adjacent to an electrical outlet. General: Review of the presented employee records revealed a staff member with hire date 11/1/2004 whose file contained a tuberculosis skin test that was interpreted on 7/3/17. Continued review of the file revealed a second tuberculosis skin test that appeared to have lines that were whited out on the date of birth, date given and date interpreted section. The year on the date of birth section appeared to be changed and was illegible. The date-given portion appeared to have a month and year number that was changed. The date- interpreted portion had a date listed where the inks did not match; the symbols used in the date did not match and a line was not consistent under the date. The name portion on the certificate had a line that was not consistent under the name and a large portion of the line was missing under a portion of the staff member’s last name. Upon request, the staff member in charge did not have a different tuberculosis skin test for the aforementioned staff member at the time of the survey. In the bathroom within the two and three- year-old room and the boy’s bathroom there was no ventilation to outside air; the ceiling vent fan blades were not visibly moving and there was no sound of the fan blades moving to circulate the air. In the girls' and boys' bathrooms, only one out of three of the toilet stalls were equipped with toilet paper, leaving the other two toilet stalls without toilet paper. Observation and interview with the director revealed thirty (30) children present during time of survey; however, forty-two (42) children were signed into the daily attendance record for that day. Interview with the director revealed the missing children on the daily attendance record had already left the child care center that morning for school on the school bus. General: Based on review of documentation and interview, the child care center failed to maintain employee records in accordance with regulations. Review of staff files revealed a staff member with the hire date of 05/09/2018 did not have proof of education documentation (high school diploma, GED, or Commonwealth Child Care Credential) on file. General: Based on observation, the child care center failed to comply with regulations regarding equipment. In the school age room there was several areas of paint chipping away from a purple shelf. Further observation revealed in the school age room a child’s blue couch that was torn at the edges exposing fabric at the seams. Further observation revealed in the school age room a zebra print area rug that was torn and frayed at the edges causing a tripping hazard. Further observation revealed in the school age room eight (8) folding chairs with stained upholstery, and four (4) wooden tables with several areas of wood chipping away from the edges. In the three-year-old room, there was a blue and red child’s chair that was torn at the edges, exposing fabric at the seams. In the Pre-K room there were several areas of paint chipping away from a red shelf, orange shelf, and a wooden doll house. Transportation Not In Compliance
General: Based on observation, the child care center failed to comply with regulations regarding outdoor play. Observation of the toddler playground revealed several broken riding toys discarded in a corner, a bucket of toys submerged in standing water, and overgrown weeds. The toddler playground was unclean and unkept. Observation in the big playground revealed several overgrown weeds, a broken blue and green push toy, a large gap of missing concrete pavement which could cause a tripping hazard, a red and blue sand table with dirt and rocks in standing water, an orange water table with dirt and rocks, a broken basketball rim, and several broken hula hoops hanging on top of the basketball rim. Review of the transportation policies revealed there was no written plan to ensure the staff perform duties when transporting children from the child care center. General: Based on observation, the child care center failed to comply with regulations regarding programming. In the two and three-year-old room, a three-year-old child was sitting at the table eating breakfast when the three-year-old child asked the staff member with the hire date of 04/14/2016 for more cereal. The aforementioned staff member replied back to the three-year-old child, “when you eat your blueberries, you can have more cereal”. Premises Not In Compliance
General: Based on observation, the child care center failed to comply with regulations regarding premises. In the toddler room the ceiling light did not have a protective shield over it. General: Based on observation, interview and review of documentation, the child care center failed to comply with regulations regarding premises. In the following rooms there were several areas of paint chipping from the wall: two and three-year-old, school age, Pre-K, lunchroom and toddler room. In the following rooms there were holes in the walls: two and three-year-old, lunchroom, and toddler room. In the following rooms there were cracked floor tiles: two and three-year-old, school age, and lunchroom. In the two and three-year-old room there was tile missing by the table and chairs. In the same aforementioned classroom there was a broken baseboard. In the school age room and the three-year-old room the baseboard was separating from the wall. In the school age room and the three-year-old room the ceiling tiles were cracked. In the two and three-year-old room and the Pre-k room the ceiling tile was not fitted and secured tightly in place causing it to partially hang from the ceiling. In the toddler room and boy’s bathroom the ceiling tiles were stained, indicative of water damage. Interview with the director revealed there has been water damage in the child care center. Further observation revealed in the toddler room the floor tiles were stained with brown markings. In the two and three-year-old room the carpet was torn and frayed causing a tripping hazard. Observation in the Pre-K room revealed the sprinkler system located on the ceiling had rusted and had corrosion on it. Review of the Statement of Deficiencies and issued by the cabinet and the Plan of Corrections submitted by the center during the current licensure year revealed many of the same regulatory violations were identified at the previous renewal survey.
General: Based on review of documentation and interview, the child care center failed to comply with regulations regarding transportation. Review of the transportation policies revealed there was no written plan to ensure safety procedures for transporting children, loading and unloading a child, and providing adequate supervision of a child. Interview with the director revealed the requested documentation was unavailable at the time of survey.

Review of the transportation policies revealed there was no record of a written document the first and last names of the children who were transported, and each time the children got on and off of the vehicle completed by a staff member. Interview with the director revealed the last field trip taken by the child care center was on 07/20/2017 to Broad Run Park. General: Based on observation and review of documentation, the child care center failed to comply with regulations regarding food service. Observation in the kitchen revealed the freezer thermometer registered three (3) degrees Fahrenheit.
General: Based on observation and review of documentation, the child care center failed to maintain food service in accordance with regulations. The stove had food debris and grease build-up along the inside walls and on the inside of the door.
General: Based on observation, review of documenation and interview, the child care center failed to comply with regulations regarding food service. Observation in the Infant revealed a container of, “Nutramigen”, and “Good Start” powder based formula with a child’s name on it. Interview with a staff member revealed the parents supply the formula and the staff member will measure, mix, and prepare the formula for consumption. Observation of the Boys bathroom revealed there was one (1) toilet in non-working order. Further observation revealed the Twos and Threes Room bathroom had no ventilation to outside air. General: Based on Observation, the child care center failed to maintain the premises in accordance with regulatory requirements. Observation of Blue Room B and the Indoor Gross Motor room revealed several areas of paint chipping and peeling away from the walls. Further observation revealed a large hole in the cement wall of the following rooms: Two and Three year old, Infants, Blue Room A, Indoor Gross Motor, and the Cafeteria. Continued observation revealed the base board was separating from the wall of the Blue Room A, Blue Room B, Indoor Gross Motor room, and Cafeteria. Further observation revealed a broken ceiling tile in the School Age Room, cracked floor tile in the Toddlers' room by the table and sink. Continued observation revealed a torn floor carpet in the Twos and Threes room causing a tripping hazard, and torn floor carpet by the dramatic play area. General: Based on Review of Documentation and interview, the child care center failed to comply with regulations regarding written documentation. Review of the attendance records and interview with the director revealed there were twenty-three (23) children signed in for the day, with forty (40) children present at the time of survey. General: Based on Observation, the child care center failed to maintain the director requirements in accordance with regulations. Observation of the entry way table revealed a bottle of hand sanitizer indicating "keep out of reach of children" on the label. Further observation revealed, in the hallway there was one (1) electrical wall outlet that did not have a protective covering, and an electric outlet in the Indoor gross motor area that did not have an cover over the electrical outlet exposing the electrical box. Review of staff files revealed an annual written performance evaluation was not presented for review at the time of survey for a staff member with the hire date of 11/01/2014. General: Based on Observation and interview the child care center failed to maintain the premises in accordance with regulations. Observation of the Boys and Girls bathroom revealed there was not a supply of toilet paper in two out of the three toilet stalls.

Interview with the director and staff member with hire date 06/09/2014 revealed they are not following the correct procedures for completing toy sanitation for mouthed toys. General: Based on Observation, the child care center failed to comply with regulations regarding food service. Observation in the Cafeteria revealed that the freezer thermometer registered four (4) degrees Fahrenheit. Further observation in the Kitchen revealed there was no thermometer in the deep freezer.
General: Based on Observation the child care center failed to maintain food service in accordance with regulations. The center failed to ensure all equipment surfaces contacting food were clean and sanitary. Observation of the kitchen revealed the stove had grease buildup on the stove door.
General: Based on Observation and interview, the child care center failed to comply with regulations regarding food service. Observation in the Infant room revealed there were two (2) boxes of formula with children’s names on it. Interview with staff member revealed the parents' of those two (2) children provide the formula and the staff member then mixes and measures the bottles for the child's daily use. General: Based on interview and observation, the child care center failed to comply with Outdoor Play Area regulatory requirements ensuring a minimum of 60 square feet per child. Interview with a staff member revealed she brought thirteen (13) children, ranging in age from 3 years to 5 years, to the small outdoor play area immediately behind the center at 9:40 AM. Surveyor observed at 10:05 AM the small outdoor play area contained thirteen (13) children and that the Director immediately removed two (2) children. The surveyor subsequently measured the outdoor play area and determined that it contained 715.99 square feet, including 76.666 square feet used for Little Tykes storage. Based upon the total square footage, the maximum capacity of the small outdoor play area is 11 children and its use upon entry, therefore, exceeded capacity from 9:40 AM to 10:05 AM.
General: Based on review of documentation, the child care center failed to comply with annual training regulatory requirements. Record review revealed a staff member with a hire date of of 12/15/14 had obtained fourteen (14) of the required fifteen (15) hours of cabinet-approved early care and education training, including her previously obtained orientation training, and did not complete required pediatric abusive head trauma training in her first year of employment. 345 - Driver Requirements Not Applicable 922 KAR 2:120. Section 12. Transportation. (18) A driver of a vehicle transporting a child for a center shall: (a) Be at least twenty-one (21) years old; (b) Complete: 1. The background checks as described in 922 KAR 2:110; and 2. An annual check of the: a. Kentucky driver history records in accordance with KRS 186.018; or b. Driver history records through the state transportation agency that issued the driver’s license; (c) Hold a current driver’s license which has not been suspended or revoked during the last five (5) years; and (d) Not caused an accident which resulted in the death of a person. General: Based on observation, the child care center failed to comply with regulatory requirements that the premises be kept in good repair. Observation revealed two (2) missing vinyl floor tiles in the lunchroom just before entering the kitchen. Additionally, the kitchen contained five (5) chipped/broken vinyl floor tiles and one (1) missing floor tile.
General: Based on observation, the child care center failed to comply with Food Service regulations that the kitchen be kept clean. Observation revealed a substantial amount of grease covering the inside oven door and oven window.
Based on review of documentation, the child care center failed to comply with children's records enrollment information regulatory requirements. Record review revealed two (2) children with enrollment dates of 07/27/15 and 01/04/16 had no preferred hospital information, and two (2) children with enrollment dates of 09/08/14 and 04/06/15 had no required physician information in their enrollment information.
General: Based on Review of Documentation and interview the child care center failed to comply with employee records regulatory requirements. Review of documentation revealed two (2) staff members both hired on 12/15/14 did not have six (6) hours of cabinet-approved orientation on file. Interview with the director revealed the aforementioned staff members have not completed orientation training.
General: Based on Observation, the child care center failed to maintain the premises in accordance with regulatory requirements. Observation in the Three’s Room revealed one ceiling tile loose creating an open space in the ceiling. Continued observation in the Three’s Room revealed that a corner of one ceiling tile was broken off. Further observation in the Indoor Gross motor area revealed one ceiling tile was missing exposing fiberglass insulation.
General: Based on Observation, the child care center failed to maintain the outdoor play area. Observation revealed that the helicopter shaped climbing apparatus was sitting on concrete and failed to have protective surfacing.
General: Based on Observation, and Review of Documentation the center failed to maintain written documentation. Observation revealed at 11:28 a.m. forty-six (46) children were present at the center. Record review of daily attendance records revealed that thirty (30) children’s arrival times were recorded.
General: Based on posted documentation review, the child care center failed to post documentation in accordance with regulatory requirements. Review of posted documentation revealed that the center failed to post current rates for child care.
