Observation of the outside play area revealed the grass was over three (3) feet tall. The grass was growing over the picnic tables. Further observation of the ground located on the outside playground, beside door number twelve (12), revealed a hole that was three (3) feet deep with a concrete block on the side of the hole that was accessible to the children. General: Based on review of documentation and interview, the child care center failed to maintain employee records in accordance with regulatory requirements. Review of the presented employee records revealed five (5) staff members with hire dates 8/26/93, 10/30/06, 8/8/16, 9/29/14 and 9/27/10 whose file did not contain documentation to verify completion of the fifteen (15) hours of cabinet approved training. Review of ECE-TRIS verified that the staff members did not complete fifteen (15) hours of cabinet approved training. Review of the presented children’s records revealed six (6) children with the same enrollment date of 8/14/19 whose file did not contain the child’s general health status and medical history including allergies. Review of the presented children’s records revealed three (3) children with the same enrollment date of 8/14/19 whose file did not contain the name of the child’s physician. Review of the presented children’s records revealed three (3) children with the same enrollment date of 8/14/19 whose file did not contain the name of the family’s preferred hospital. General: Based on Interview and Review of Documentation, the child care center failed to maintain adequate supervision in accordance with regulations. Interviews with multiple staff members revealed on 12/18/15, a one-year-old walked out of the classroom and into the hallway unsupervised when either a teacher or parent opened the classroom door. Continued interview with staff members revealed the one-year-old was left unsupervised in the hallway for an undetermined amount of time, ranging from less than one (1) minute- two (2) minutes, before being returned to her classroom by either a teacher or the child’s mother. General: Based on observation in classroom 105, there was a crib with a toy attached to the inside of a crib. Based on observation in classroom 107 there were five toys attached to the inside of five separate cribs. Based on observation in classroom 105, mobiles were attached to cribs that were used for children that were able to sit up and pull up onto their knees.

General: Based on Observation, the staff person in the infant room was feeding a child who was sitting in a bouncy seat while at the same time feeding another child in her arms. Further observation revealed that the staff person then placed one child's bottle in the bouncy seat with the other child when she was finished. Further observation of two year old room and two infant rooms revealed cords from CD players accessible to the children.
General: Based on observation, the child care center failed to maintain equipment in accordance with regulatory requirements. Observation of a white wooden building located on the outside playground revealed the wood on the lower part of the building was rotted, leaving the wood splintered, and white paint was chipping off of the building that was accessible to the children. Observation of the door attached to the white wooden building revealed there was a one (1) inch metal plate on the bottom of the door that was rusted and bent that was accessible to the children. Continued observation of the door on the white wooden building revealed the bottom of the door frame was rotted, missing pieces of wood and a two inch (2) rusty nail was protruding from the door frame that was accessible to the children. General: Based on review of documentation and interview, the child care center failed to comply with background checks in accordance with regulatory requirements. Review of the KARES Report revealed two (2) individuals listed as current employees who were determined to be eligible for employment on 5/16/18 and 6/12/18. Interview with the staff in charge revealed that the aforementioned individuals were no longer employed with the child care center. Based on observation, the center was not consistently using attendance records to sign children in and out of the center on a daily basis using attendance sheets that are required in accordance with 922 KAR 2:160, Section 13 as some of the children in attendance at the child care center receive services through the Child Care Assistance Program.
