Observation of the girl's bathroom revealed the air vent, located on the ceiling directly above the toilet, was covered with dust. General: Based on observation, the childcare center failed to maintain an outdoor play area in accordance with regulations. Observation of the outdoor play area revealed three (3) large gaps/holes between the wooden perimeter privacy fence and the ground, thus creating multiple entrapment hazards. Further observation of the outdoor play area revealed a large section of black weed barrier protruding up from the base of a tree. Continued observation of the outdoor play revealed a large tree branch lying on an adjacent sidewalk that leads to the access gate of the outdoor air conditioning and heating units. The branch was within reach of students as they enter the play area. The sink in the Dr. Suess room failed to produce hot water at the time of the survey. During the inspection, the surveyor ran the water for several minutes and it never warmed to proper temperature.
