On 12/29/25 a two year old child was left without supervision when they walked away from their class and down another hallway without a caregiver as the class moved to the indoor play area.
On 12/29/25 Efficient supervision was not followed during the transition from the Two's classroom to the indoor play area, threshold count was not completed when arriving at the indoor play area.

At the time of inspection items labeled Keep out of Reach of Children were found accessible to children in the following areas: PSII-had dishwashing liquid, Swiffer wet cloths, Shea lotion, and Sanitizing spray under the sink in an unlocked cabinet. SA A had Goo Gone, dishsoap, Borax and sanitizing spray in an unlocked low cabinet. Twos had Swiffer wet cloths, Disinfecting wipes in an unlocked cabinet under the sink.
At the time of inspection the SA A had a toilet brush in the bathroom. The Tod room and the Twos room had Swiffer mops in their bathrooms, not sanitary.
At the time of inspection there were toys on the changing table in the Toddler and Twos classrooms.
At the time of inspection the children in the PSII classroom were watching a non-educational movie while eating lunch.
In the kitchen there was a bowl of cut cucumbers not labeled with the date. In the Twos classroom there was 2 baggies of snacks not dated.
At the time of inspection the children's coats in the PSII classroom were touching.

On 8/29/25 a staff member did not help a child use the restroom while child was being potty trained, resulting in the child being soiled several times during the day.

At the time of the inspection the playground with 12 children ages 18 months to 36 months was over maximum group size of 10 for the youngest child.

Hazardous items and items labeled Keep out of Reach of Children were found accessible to children in the following areas: PS2 had diaper ointment, baby powder, butt paste and chapstick in low diaper drawers. In the SA room there was Goo Gone and fabric care under the sink in an unlocked cabinet. In the PS1 room there was disinfectant spray and sanitizing spray under the sink in the unlocked cabinet.
In the PS2 room, SA, TodA, PS1 had paper towels sitting on the counter, not on a dispenser. Inf B is storing paper towels open under the sink. In the PK room there was toilet paper on top of the dispenser in the bathroom.
In the Inf B room there were 3 cans of formula with no dates, 1 container of cereal with no date, in the fridge there was 1 container of yogurt with no date.
No thermometer in the infant B fridge or the Freezer in the kitchen.
In the SA room the wall covering is coming off the wall. In the PK bathroom there is holes in the wall, not sanitizable.
In infant room B there was 1 bottle not labeled with date or contents.
In infant room A there were 3 uncovered bottles in the fridge and 2 uncovered extra bottles in cubbies.
In PS1 the carpet edge is lifting, not secured.
