COMPLAINT: Director confirms 1 child was unsupervised when teacher transition the class from the gym to the classroom the child did not go into the classroom and was fond in the lobby. Director confirmed 2 children were unsupervised for about 1-2 minutes when they left the playground, came into the building and when into the gym. Children were located in the gym. Director confirms 5 children were unsupervised when they left the classroom and went directly outside. One child was left on they playground unsupervised for an undetermined amount of time.
COMPLAINT:COMPLAINT: Program was out of Group Size when they had 12 children ages 1 and 2 years of age on the playground. Group size for children 1 year old is 2:10.
Center failed to report serious injury of a child to the division.
One staff member present needing new Adult background check by the Federal Bureau of Investigation (fingerprints) as she is now 18 years old.
Complaint 31979: The consultant reviewed the CSR for the two weeks. Infant 1 out of Ratio on 1/21 with 8: infants Toddlers 1 out of ratio on 1/23 with 8:1 ratio, 1/28 out of ratio with 8:1 Pre School 2 out of ratio on 2/5 with 13:1 Complaint 32007 : Consultant reviewed the CSR’s for the week: Pre School 2 out of ratio on 2/5 with 13:1 Ratio was not maintained, due to the one child getting off the bus and being added to the class.
Complaint 31979: Director shared this did happen during naptime. Mr. Alex admitted this did happen. Due to Mr. Alex asleep in the classroom, supervision was not maintained.

Toddler restroom contained a dust pan on the floor and toy cars on a shelf. Preschool 2 had clothing and toilet paper on shelf. PreK 4 restroom contained a toilet brush.

Toddler 1 and Toddler 3 had minimal toys/lack of duplicate on shelving units for toddlers to select from. Some shelving units were empty at time of visit.
Discovery 1 and Discovery 2 did not have art materials out for children to explore.
At time of visit, toddler caregiver did not wear apron or hairnet while serving breakfast.

Items labeled "Keep out of reach of children" were accessible in the following classrooms: Infant 3 and Infant 4 changing tables with a detterent (not a lock) contained butt paste, A&D ointment, Aquaphor, Aveeno sunscreen, and Desitin. Preschool 1 and Preschool 2 classrooms had unlocked cabinets that contained caregivers' purses. Lobby had handsanitizer on an open shelf accessible to children.
At time of visit, Infant 3 and Infant 4 missing infant feeding plans posted in the classrooms.
