Each child has age appropriate immunizations including Varicella and Pneumococcal vaccines. Documentation includes: -Attendance records of all children in attendance. Immunization records for each child (includes month, day and year given for each immunization and childs birth date).
At time of visit, a 5 month old infant was sleeping in a crib wearing a hooded sweatshirt.
HR, caregiver had expired fingerprints in staff file.

Green room: Hand sanitizer and disinfectant wipes labeled “keep out of reach of children” are on white plastic drawers & Windex under the sink; all accessible to children which is unsafe. Green Room: the Carpet is unraveling and pulling up which is a tripping hazard. Orange room: Under the window there is peeling paint, exposed drywall and the baseboard is coming off the wall which is unsanitary. Peeling paint throughout the room which is unsanitary. Purple (large room): Peeling paint throughout the room which is unsanitary. Small purple room: Peeling paint throughout the room which is unsanitary. School-age hallway: The air vent in the ceiling is covered in dust which is unsanitary. School-age hallway leading into girls bathroom the carpet is pulling and coming up which is a tripping hazard. School-age room: Peeling paint throughout the room which is unsanitary. Toddler Blue room: Baseboard is off the wall and there is peeling paint throughout the room which is unsanitary.

Girls restroom in the side hallway: seal is cracked along the wall and round the sinks. Boys restroom in the side hallway: seal around the sink is cracked along the wall and sink. This is not sanitary. There is a toilet plunger in the corner of the restroom. This is not sanitary. Restroom by the back door, sink seal has large cracks along the wall and counter. There is a hole in the wall that is half taped over in the left stall. The middle partition has tape pilling away from the partition.
