Director stated management confirmed the parent concern by watching about 2 hours of video. The staff in In fant room 1 was not the regular staff for the room that day and was dismissed from her job with the facility immediately. Caregiver did not follow the infant's individual feeding plan by by not providing 2 of the bottles.

There are Dents in walls and peeling paint which is unsanitary in the following rooms: Preschool 3, PreK, Gross Motor room, Toddler’s 2, Toddler’s 1, Two’s 2, 2.5’s, Infant 1 and Infant 2.
