On 9/4/25 an unqualified caregiver walked 3 children down the hall to another classroom when they ran out the front door of the building with out the caregiver, leaving them without any caregiver supervision. Unqualified caregivers cannot be left alone with children, therefore the children were not being supervised in the hallway.
On 9/4/25 the provider did not have documentation of a criminal history check for the caregiver (ZD) involved in the above incident. At the time of inspection, 2 caregivers (FP, SP) had expired fingerprints on file with the division. Send notification to consultant of consent form submission to update fingerprints.

Room 20 class was left out of ratio when 15 children with 1 qualified caregiver and 1 unqualified caregiver were in the music room.
On 9/26/23 a caregiver used inappropriated discipline when they threw a child's shoe back at child after the child threw it at them and hit them in the head.
At the time of the reported incident the caregiver's Child Abuse and Neglect training was expired.
