At the time of inspection the toddler room did not have paper towels in the dispenser, the older 2's room had 2 rolls of paper towels sitting on top of the dispenser, the 5's room had paper towels on top of the dispenser, the School age bathroom had no paper towels, and 2 rolls of open paper towels on top of a cabinet.

At the time of inspection the infant caregivers were not following Safe Sleep practices when one 6 month old infant was found sleeping in a bouncy seat.

Unless the provider is a parent, stepparent, quardian, custodian, or other relative to each child in the care of the provider, the individual annually must recieve at least twelve (12) hours of continuing education approved by the division and related to the age appropriate education development, care, and safety of children.
At the time of inspection there was paper towels not on dispensers in the infant room, Cafe, and 4/5. There was no paper towels or soap at the sinks in the SA bathrooms, and at the toddler sink.

At the time of inspection the changing table in the 2/3's room does not have 2 feet all the way around it as sanitizable space.
At the time of inspection there were brooms and dust pans in the following bathrooms: infant, toddlers, 2/3's. In the 4/5's bathroom there was several containers of toys, and teachers supplies stored in the bathroom.

Upstairs hallway: outlet covers. Downstairs hallway: Electrical room was unlocked with 6 bleach bottles on the counter, Furnace room was unlocked, and there were 10+ gallons of paint in a closet across from the electrical room, not locked. School Age: outlet covers in a power strip, paint peeling.

The provider shall maintain a written policy requiring an individual providing a criminal history to report any criminal convictions to the provider.
