Room 6: Plunger stored in restroom. Room 7: Toilet brush stored in the bathroom. Bathroom used as storage, Basket of books, a box of open diapers. Toilet paper not on dispenser. Room 8: Toilet plunger stored in bathroom All items in bathroom must be sanitizable.
Room 1 Toddler room: floor tiles along the walls and the outside door are cracked and missing pieces. Room 5: Base trim along the wall is missing in places and buckled and pulled away from the wall in other places throughout the room. Indoor playroom: floor tiles are cracked and missing pieces along the walls. Hallway: Floor tiles are cracked and missing pieces along the walls.

Room 7: sink seal is cracked creating a gap between wall. Toilet brush stored in the bathroom. Room 8: Sink seal is cracked. The sink countertop and sink base does not line up creating a gap.
Staff did not use positive discipline, getting down to the child’s eye level and communicating with the child.
