At time of visit, the following restrooms had broom sets, toys and chairs being stored in Large Motor, and Infant 1. Toddler restroom toilet had a large piece of cardboard covering the toilet bowl.
At time of visit, Caregiver was observed yelling and stating to school age children, "You all are acting like preschoolers. It's not cute , listen and clean up." Caregiver continued yelling instructions for school-age children to comply.
At time of visit, Probationary license and settlement agreement is not prominently posted in a place viewed by parents as instructed under agreement.

Ratio following the youngest child is 5:1. During this time in the lobby, one 3 year old went back to classroom to use restroom without a caregiver, ratio 1:0 Ratio for 3 yr old is 10:1. During visit consultant observed the toddler class out of ratio with 11 toddlers with 2 caregivers. Ratio for toddlers is 10:2. During visit consultant observed the twos class out of ratio with 11 two year olds with 2 caregivers. Ratio for two year olds is 10:2.

Items labeled "Keep out of reach of children " were accessible to children. Toddler 1 - caregiver personal bags on bottom shelf in unlocked cabinet. School age- caregiver purse on shelf within reach of age group in unlocked cabinet. Riser room- unlocked, contained bleach gallon and floor cleaner. Hallway- door open containing 4 gallons of bleach, 4 sink detergent cleaners, 4 gallons of lime-a-way and 5 gallons of floor cleaner. Pantry door open- Ecotemp Ultra Klene 6 gallons and Dawn Soap. Kitchen door open- 1 gallon of bleach and 2 bottles of Dawn soap.

The preschool class out of ratio with 13 children ages 3 years to 5 years with 1 qualified caregiver on 5/29-5/30 time of complaint and consultant observed the preschool class out of ratio with 19 children ages 3 years to 5 years with 1 qualified caregiver on 6/6/24. The twos class out of ratio with 8 two year olds with 1 qualified caregiver on 5/28/24 documented on CSR.

Caregivers in the infant and preschool room are unable to communicate developmental progress of children, including verbal skills in English.
Complaint on 1/16/24, infant diaper was not changed based on infant's individual schedule. Kinder app showing infants are being changed all at the same time and not based on individual child's schedule.
