On 4/2/26 a 2yr old was without the direct supervision of a caregiver when the class and caregivers went inside and left them on the playground.
In the fridge there was a bag of carrots not sealed closed or dated. In the pantry there was a bag of noodles not sealed closed or dated.
On 4/2/26 a 2yr old was left out of ratio when the class and caregivers went inside and left them on the playground. Ratio on the playground at the time was 0:1, Ratio for the two year old age group is 1:8.

PK room had basket of sunscreen under the sink in an unlocked cabinet.

COMPLAINT: Supervision was not maintained in the 3’s classroom when 2 children were found by cargiver, unattended in the bathroom undressed.
