Director self reported on 4/7/26- Video of caregiver grabbing a 2 1/2 yr old child's arm restricting movement, hitting child with her hand on the arm. Caregiver is heard on video yelling at the child. Director self reported on 4/6/26- Caregiver grabbing a 6 yr old child's wrist to move from one location to the other in classroom.
In the infant room Director self-reported that staff fell a sleep with 2 infants in room asleep. The rule states person must be directly engaged in supervising and implenting activities for children to be counted for ratio. (2 children 0 staff)

In the school age room the back restroom had a plunger and in the Toddler 2 shared restroom a mop, bucket and dust pan were in the restroom. These items are not sanitizable.

A 3 yr old child was left in the kitchen area of the classroom unsupervised until another staff member found him doing a room check after a seizure incident had happened earlier. This left the child unsupervised for 2 to 3 minutes.

A 5 year old child was unsupervised in the classroom for about 30 seconds, when staff left and took the other children to get some tape in office area and returned to class.
Facility was out of child/staff ratio when a sleeping child (5years old) was found by another staff member in the classroom alone.
