In Disc Pre sch room 2 outlet covers were missing, in the 5 yr old room on south wall an outlet cover missing and in boys upstair RR an outlet cover missing, which is a safety risk for children.
In toddler B restroom two swifter type tools were stored in room. In the upstairs Girls and boys and downstairs girls and boys papertowels and toilet paper rools were stored on top of paper towel dispensers. This is not properly dispensed and items are not sanitizable.

On May 15, 2025, a staff member was correcting a misbehvior by a 5 year old shild and shoved the child. The child lost her balance and fell resulting in a soft-tissue injury. Both staff members failed to report the incident and were dismissed for not following Center policy.

on 1/8/2025 the buses did not start after being out for two days due to weather. It was 7:15 am and the Director made the call to have staff take the children to school that is one mile away. the staff used their personal vehicles and parents had not given permission, Parents were notified after this had been completed.
