During inspection feeing plan for 7 week old infant was missing
At the time of inspection a child in the two's room did not wash their hands after their diaper was changed.
Hazardous items and items labeled Keep out of Reach of Children were found accessible in the following areas: In the toddler room there was a container of Vaseline in a cabinet under the changing table behind a child deterrent. In the Infant room there was a teacher's purse in a low cabinet behind a child deterrent. In the 3/4's room adult scissors were on a low shelf accessible to children.
At the time of inspection the caregiver in the two's room did not wash their hands after changing a diaper.
At the time of inspection the toddler bathroom did not have toilet paper.
Hot water at all the hand washing sinks was over 120 degrees F, except at the school age bathrooms where is did not reach 100 degrees F.
The infant freezer read 4 degrees F at the time of inspection.
10 children's files reviewed were found to be missing the following information: 7 missing date of admission 1 missing legal birth documenation 1 missing signed emergency authorization 1 missing a physical 8 missing docter/dentist information 8 missing developmental information. In the toddler room the cereal was placed directly on the table, no napkin or plate was used which does not follow food service practices.
Dog rug in the infant room does not have adequate anti-skid material backing to prevent the rug from moving.
In the 3's room and the 4's room there are paper towels out, not on a dispenser. In the toddler bathroom there was a ball, mat and the tunnel mat was draped over the wall.
Both 2 year old rooms are missing the required climbing equipment.

The big playground equipment has missing bolts, items are taped together. A leg of a piece of equipment is broken open and sharp edges. The small playground has a toy flower that is broken and has sharp edges on the mushroom.
3's room is missing literacy area with books and is in need of additional toys to avoid conflict in the classroom.

In the PreK room there was an unlocked cabinet with a purse, lysol, glass cleaner, disinfectant spray and clorox wipes.
3 Infant feeding plans were not up to date. 1 infant does not have physician letter permitting the use of powdered formula instead of ready to feed. 2 physicians letters for powdered formula are missing medical purpose for use or the words "powdered formula" instead of the ready to feed formula.
This licensing inspection recorded no violations or advisories.
