On 1/6/2026 at 11:40am staff member Megan Kilander refused to give a 4-year old female child seconds of lunch and removed her entire plate and cup of milk because the child was 'playing' in her applesause.

Director self-reported that on 9/8/2025 a two year old male child was unsupervised for 60-90 seconds.
Rooms 1, 2, 4, and 5 did not have two (2) feet of sanitizable surface surrounding the diapering tables. Several diapering tables had carpeted steps which are not sanitizable.
Items labeled 'Keep out of reach of children' accessible as follows: -In Room 2 there was an unlocked door to the laundry room containing detergent on a low shelf; -Room 8 had an unlocked cabinet with Quat and Aveeno lotion.
Snack items in Rooms 3, 4, 5, 6, 7, 8, 9, 10 were not labeled with date opened.
At time of visit, no phones in program had posting of name of program, fire department, police department, ambulance, nearest hospital, poison control, county child protective services, and child care information line.
