The facility shall maintain, and make available to the department current records and information including: 17-895.1-71 Disaster plan for emergencies. (c) The facility shall complete practice drills for the various emergency situations at regular intervals, or whenever a new procedure has been added.<br/> <br/> Provide us with your current school year log of emergency drills log.<br/>
1. Provide photographic evidence (wide angle photographs, so we are able to confirm the location of the items) of the physical space for infants and toddlers who become ill after arrival at the facility.<br/> <br/> 2. Provide written attestation that your site has an operating telephone (including telephone number) in case of an emergency that is accessible to all staff<br/> <br/> 3. Provide us with your plan for accessing a telephone when child care is provided somewhere other than in the usual location where care is regularly provided<br/>
The facility shall have written policies, which have been developed with the assistance of the facility's health consultant and that minimally require:<br/> <br/> (4) No smoking, including electronic smoking devices, is allowed in the presence of the infants and toddlers, nor in any parts of the building, which are used for child care, during the hours of child care operation, and personal nicotine products shall be stored out of the reach of children.<br/> <br/> (6) Medication that impairs or limits your staff member's ability to provide care shall not be consumed prior to or during the hours of child care<br/> <br/> Please update your employee handbook /policies and provide a copy after you have updated it.<br/>
Please provide a written attestation that all cleaning materials are clearly labeled and stored in a secure area that is away from food preparation areas.<br/>
Please update the disaster plan to cover emergencies such as fire, flood, or natural disaster (tsunami) or other emergencies, which shall include procedures for evacuation, relocation, sheltering in place or lock-down, communication and reunification, continuity of operations, accommodations of infants and toddlers, and providing for the needs of children with disabilities, special needs, and chronic medical conditions.<br/>
Provide us with a copy of your current parent handbook with the required additions and/or edits.<br/>
Provide photographic evidence (wide angle photographs so we are able to confirm the location) of a list of food items that are provided by the caregiver posted in a prominent place, a location that is easily noticeable or seen.<br/> <br/> If the menu is share with parents electronically, you are still required to post it in a prominent place.<br/>
Safe Sleep All staff must have safe sleep training. Please have XXXXX complete their safe sleep training immediately.<br/> <br/> <b>*Safe Sleep training for XXXXX, who was hire in September 2024 has not been completed. Please have XXXXX complete the required annual safe sleep training by November 30, 2024. Failure to do so will result in XXXXX being excluded from providing child care until the training requirement is met.</b>*<br/>
Staffing patterns During our visit, we observed that the program meets staff-child ratio. However, the staff sequence is not met in two classrooms as follows -<br/> <br/> Infant A: 6 enrolled; 3 present with XXXXX (AQ)<br/> <br/> *Please inform Worker when there are adequate qualified staff to meet this requirement.<br/> <br/> Room C 1 year old: 9 enrolled; 5 present with XXXXX ( qualified through 01/31/25- approved Demonstration Project) and Cheyenne N (AQ)<br/>

However, the staff sequence is not met in two rooms; room Infant A and Room C.<br/> <br/> Infant A: 6 enrolled; 3 present with XXXXX (AQ)<br/> Room C 1s: 9 enrolled; 5 present with XXXXX (AQ) and XXXXX (AQ)<br/> <br/> On 4/22/2024, Worker received a new staff sequence form reflecting updated staff coverage.<br/> <br/> Note: There currently is an approved Demonstration Project that waives the qualification requirements for staff member XXXXX that will expire on May 31, 2024. Please submit a new request for this to be waived for the new licensing period effective June 1, 2024.<br/>
