Admission policies that require sufficient information and instruction to be received from parents/guardians that enable caregiver to care for the child Worker inspected child records. The records are incomplete and inconsistent with filed documents.<br/> <br/> Please make sure that child records shall include at minimum, the following:<br/> <br/> 1. Information and instructions to care for the child<br/> 2. Emergency contact information of person to assume responsibility when parents are not available<br/> 3. Contact information of other persons who can pick up child<br/> 4. Health information - physical exam, TB clearance and immunization information<br/> 5. Signed permissions<br/> <br/> <b>We will allow time for you to complete the digitization of the children's records and will return to inspect the children's records at a later date. Once we have inspected the children's records and it meets requirements, this deficiency will be corrected.</b><br/> <br/> <br/> You shall also maintain an emergency binder to include the signed parent permission forms for emergency care, emergency contact information and who can pick up the child.<br/>
