Operating accessible telephone Provide us with written attestation that your site has an operating telephone (including telephone number) in case of emergency, that is accessible to all staff.<br/>
Emergency telephone access Provide us with a plan for accessing a telephone when child care is provided somewhere other than the usual location where care is regularly provided.<br/>
Fire extinguisher Provide us with photographic evidence (not close-ups but wide-angle photographs so we are able to confirm the location) of an unexpired ABC multi-purpose type fire extinguisher for every room where infants and toddlers are cared for.<br/>
Log of emergency preparedness drills Provide us with your current school year log of emergency preparedness drills.<br/>
Physical arrangements for sick infants and toddlers Provide photographic evidence (wide angle photographs not close-ups so we are able to confirm the location of the items) of the physical arrangements for infants and toddlers, who become ill after arrival at the facility.<br/>
Written disaster plan Include the following in your disaster plan if not already included:<br/> <br/> 1. If you do not have written procedures for flooding, please add one;<br/> Example: If instructed by civil authorities to evacuate or if water begins to rise around the facility, we plan to gather the survival (emergency) kits and immediately move to higher ground at: <u>_</u>_<u>_</u>_<u>_</u>_<u>_</u>___<br/> <br/> 2. Accommodation of infants and toddlers during an emergency;<br/> <br/> 3. Providing for the needs of children with disabilities and children with special needs during an emergency; and<br/> <br/> 4. Providing for the needs of children with chronic medical conditions during an emergency.<br/>
Personal health habits of staff Provide us with your current Employee/Staff Handbook which includes the following:<br/> <br/> 1. Electronic smoking devices is not allowed in the presence of the infants and toddlers, nor in any parts of the building which are used for child care, and personal nicotine products shall be stored out of reach of children; and<br/> <br/> 2. Medication that impairs or limits your staff member's ability to provide care shall not be consumed prior to or during the hours of child care.<br/>
Emergency exits Provide us with photographic evidence (preferably not close-ups but wide-angle photographs so we are able to confirm the location) of all emergency exits in the facility, so we may ensure it is unobstructed.<br/>
This annual/biennial licensing inspection recorded no violations or advisories.
