One caregiver with current pediatric first aid certificate and child CPR during operating hours There is currently only one staff member listed with a valid CPR training certificate.<br/> <br/> This is insufficient because the two rooms are located across campus from one another, and it does not account for situations where that staff member may be absent or off site. Additionally, the identified staff member is not present during all operating hours.<br/> <br/> Please provide documentation showing that at least two staff members with current CPR certification are present and available during all operating hours.<br/> <br/> <br/> Also, the staff for the kindergarten prep classes during the summer shall be current with this training.<br/>
Annual background checks Annual background checks will be expiring for the following staff:<br/> XXXXX<br/> XXXXX<br/> <br/> Please have these staff turn in completed DHS 948 (Consent to Release for Background Clearances) form.<br/>
Annual health and safety trainings HAR chapter 17-892.2-35 Staff Training (e) (1) - each caregiver staff member shall complete at least 16 hours of department approved training annually... and (2) substitutes shall complete at least 10 hours.<br/> <br/> In reviewing if this requirement will be met, it was found that several staff has either one or no hours recorded and their training periods will expire between August to November 2025.<br/> <br/> <b>Update:</b> <b>Training information has been updated for many staff members. However, three remaining staff members are not meeting this training requirement: XXXXX.</b><br/> <br/> Child Care Licensing Worker Seng Lee-Ray provided the director the service summary report which shows the hours recorded and training period for each staff person via email.<br/> <br/> <b>We are reminding you that all caregiving staff need at least 16 hours of department approved training on an ANNUAL basis in at least two topic areas and substitutes need at least 10 hours of training. If they do not complete this requirement by October 31, 2025, we will have to exclude these staff members and they will not be able to count towards the staff-child ratio until they do, which may affect your license to operate a Group Child Care Center (GCC).</b><br/> <br/> Please provide the training certificates (for ALL staff) to PATCH to be recorded. As a reminder, PATCH processing time for certifications is eight (8) weeks. Please plan accordingly.<br/>
Annual background checks According to Hawaii Administrative Rules (HAR) chapter 17-892.2-3 (d) Annual background checks shall be conducted.<br/> <br/> Background checks for XXXXX is expiring wiithin 30 days. This staff person shall turn in a completed DHS 948 (Authorization for Background Check and to Release Information) form so that the department can conduct the background checks. This form has been emailed to the director.<br/> <br/> When we receive the form and all background checks are cleared, this requirement shall be met.<br/>
Annual health and safety trainings Caregiver staff must complete 16 hours annually after the initial training in at least two of the approved topic areas; substitutes must complete 10 hours.<br/> <br/> Annual health and safety training has expired, or will expire for the following employee(s):<br/> XXXXX<br/> <br/> They completed 15 hours of the required 16 hours.<br/> <br/> <b>While only the above staff is mention, we are reminding you that health and safety training is a requirement for ALL staff members. Please submit completed training certificates to PATCH so that the training hours can be recorded in the system. Staff who do not meet this requirement may be excluded in the staff-child ratio, which may affect your license to operate a Infants & Toddler program.</b><br/>
Staffing Sequence According to HAR chapter 17-892.2-36 (e) (2), staff employment sequence shall be maintained at all times according to ratio charts as provided in the Hawaii administrative rules.<br/> <br/> For 4 - 5 years old group, the first 16 children must be led by a teacher qualified person, and the second 16 children may be staffed with an aide qualified staff.<br/> <br/> Effective August 1, 2025, the staff sequence will not be met from 2:30p-5:30p since staff member XXXXX teacher waiver has expired. Either when this staff member becomes teacher qualified or another staff member assigned to this group is teacher qualified, this requirement will be met.<br/>
Transportation arrangements Please clarify if the Summer Kindergarten students are being transported anywhere during any time of the day.<br/>
Others Please update the parent handbook in accordance to chapter 17-892.2-11 Statement of Operation policies to include the following:<br/> <ul style="margin: 0 0 0 16px;"> <li>Grievances which shall include the department's information for parents to file a complaint - include Child Care Licensing Honolulu unit's phone number: 808-587-5266;</li> <li>Include a non-discrimination statement. The Department recommends this sentence: It is this center's policy to not discriminate against persons with disabilities on the basis of disability, and to provide children and parents with disabilities an equal opportunity to participate in the center's programs and services, in compliance with state laws.</li> </ul>
Public beaches, swimming activities and wading pools According to page 11 of the parent handbook, it states that summer kindergarten students may enroll in afternoon summer adventure programs, which may involve swimming, boating and/or water play. It also states that if the event is taking place on a boat surrounded by deep water (versus the beach or a swimming pool with shallow ends), any student interested in swimming must pass a 50 yard swim test administered by the lifeguards; for those who don't pass, they must wear a life vest at all times when in the water.<br/> <br/> <b>This is a reminder that children enrolled in the Summer Kindergarten Program must remain under the coverage of the Group Child Care (GCC) license for the entire operational day. If these children participate in the Summer Adventures Program, all GCC rules and regulartions continue to apply. This includes:</b><br/> <br/> <b>1. Supervision by a Hawaii DHS Early Childhood Registry staff member(s) at all times.</b><br/> <br/> <b>2. Full compliance with the required staff-child ratios, with no exceptions.</b><br/> <br/> --------------------------------<br/> <br/> <br/> <b>The department has concerns regarding the inclusion of allowing children to engage in boating or swimming activities that requires children to pass a 50 yard swim test. Children enrolled in the Junior Kindergarten Program are operating under the Group Child Care (GCC) license and not the Before and After School license; which is what the Summer Adventures program operates under. Given the developmental stage and safety considerations for this age group, it is highly recommended that children enrolled in the Junior Kindergarten Program do not participate in any boating activities or any other water activities that requires them to pass a 50 yard swim test. Therefore, we are recommending that these activities be omitted from the parent handbook.</b><br/> <br/> With any swimming, public beach or wading pool of water depths of 24" or more, the program shall provide the department with the certificates of the certified lifeguard PRIOR to the activity and they shall be on duty at all times when swimming activities or swimming pools are in use.<br/>
This drop-in / follow-up inspection recorded no violations or advisories.
Specific hours of operation It was identified on page 5 of the Junior Kindergarten & Summer Kindergarten Parent Handbook that the stated hours of operation for the Summer School program are from 7:55 a.m. to 2:30 p.m. Additionally, it is noted that Junior Kindergarten students are not eligible to participate in the After School Enrichment Program (ASEP).<br/> <br/> However, on page 11 of the PH, under the Field Trip section, it states that Summer Kindergarten students enrolled in the Summer Adventures programs (which is a Before and After School licensed program) adhere to the swimming, boating and water play expectations listed below.<br/> <br/> <b>We would like to remind you that children who are enrolled in the Summer Kindergarten Program are, and must remain, under the coverage of the Group Child Care (GCC) license at all times.</b><br/> <br/> <b>When these children participate alongside children in the Summer Adventures Program, they are still required to be covered by GCC rules and regulations throughout the entire day. A Hawaii DHS Early Childhood Registry qualified (through PATCH) staff member(s) must supervise these children at all times, and the required staff-child ratio requirements must be maintained without exception.</b><br/> <br/> <br/> ------------------------------------------<br/> <br/> Please clarify this information in the PH.<br/> <br/> ------------------------------------------<br/> <br/> <b>Also, if children from the Summer Junior Kindergarten program are participating in the Summer Adventures program, the department will need the names and ages of those children as well as the names of the of the identified qualified staff members that are supervising the Summer Kindergarten children who will be participating in the Summer Adventures program.</b><br/>

Written plan approved by fire inspector, health consultant or Red Cross The disaster plan for emergencies shall include written procedures for providing for the needs or children with disabilities, with special needs, and with chronic medical conditions during an emergency.<br/> <br/> Please provide a written plan which includes this procedure.<br/>
Sanitation inspection The Department of Health sanitation inspection expired in April 2025. Please provide the current green pass placard and inspection report from the Department of Health.<br/>
