Licensing Specialist observed several personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. Director had staff complete the updated document, putting the provider back in compliance for this standard at time of inspection. Licensing Specialist observed one child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. Director informed that the CCP has an exam date for the 8th or 9th of March, so additional time was given for the provider to come back in compliance for this standard. T/A Given regarding CCP completing their training (40) within the 15 month hire date

Supervision in the 34-year-olds group was inadequate in that a child was left unsupervised on the playground for 28 minutes. Technical assistance was provided that the protocol in place name to face should be followed by staff at all times

A maintenance closet was found to have holes in the ceiling which animals or insects could enter the facility. The director reported that this issue could be rectified by 11/13/2023
Children were napping at the time of inspection in the two and 3 year old rooms in front of the exit door. In the infant room their was also a crib in front of the exit door. This was completed at the time of inspection . The teachers moved the children to a different location away from exit doors and paths
The base of fence in several area's was loose and had jagged edges which could be harmful to a child . The director reported that this issue could be rectified by 11/13/2023

On 04/27/23, a teacher in the one year old room was found unresponsive in the classroom in front of the children at the child care center. Paramedics were called and the teacher was given Narcan transported to the hospital. The teacher admitted to another agent of the department that she she had narcotics on her person and went into the bathroom and ingested the narcotic. The teacher also admitted to the other agent of the department that she had been under the influence of narcotics while working at the facility previously. The operator, employee or substitute was supervising children while using or under the influence of narcotics, alcohol or other drugs that impaired the individuals ability to provide safe child care. The teacher admitted to another agent of the department that she had narcotics on her person at the childcare facility and did use and ingest the substance in the bathroom of the facility.
