• a non verbal autistic child was left unattended for an unknown length of time with no staff supervision. There were open spray paint cans and open cabinets with hazarous materials which was accessable to the child. The child was also observed on the floor eating a mixture of nuts and fruits which were not her own

• On July 20, 2023, the daycare employee grabbed Elijah by the arm and carried him across the room due to him urinating himself. The daycare employee appeared annoyed and this act was done out of aggression. During the incident, Elijah was twisting and was in pain. Later, when the father picked Elijah up, he stated his shoulder hurt. Friday 07/21/23, the mother took Elijah to the doctor, and everything was okay, and Elijah was only sored for a couple of days. The daycare teacher was terminated from her position at the facility due to this incident. • Completed at time of inspection

11-04 · Child Discipline

On July 20, 2023, the daycare employee grabbed Elijah by the arm and carried him across the room due to him urinating himself. The daycare employee appeared annoyed and this act was done out of aggression. This action could have caused injury to the child and could have pulled the child's arm out of socket. The child could be heard crying while his wet clothes were being taken off of him and placed in a bag. The teacher left left the child in the room by himself and walked out of the room and walked out of the line of sight and supervision of the child. The teacher did come back in the room and the child followed her back outside. while in the bathroom, the teacher could be heard berating the child for urinating on himself
