Complaint alleges that when parent went to pick up his son at 3: 00.pm, age 8, from summer camp, the staff could not locate his son. It took them 30 minutes and they still could not locate his son. They reached out to another teacher, who was on a field trip in West Palm Beach with 6-year-olds, who they described the child to that individual. The parent did not authorize his son to go on this, or any, field trip. One of the owners called the father to explain to him what happened after his staff informed him of what was happening. Parent was informed that there was a mix up with the name because they have another student with the same name. The child did not return from West Palm Beach until 5:30 pm. INVESTIGATION: A complaint inspection was conducted on 6/16/2025 and a statement was received from the office manager and the assistant director. Per the assistant director (K.B.N.), she stated that she was not on-site at the time the parent arrived to pickup the child. She received a call from the office informing her that the parent was there to pick up a child who was not at the center. She proceeded to call a fieldtrip staff member that was with the group. She stated that she described him to the staff member and the staff member located him and sent her a picture of him. After he was located, she had the assistant director Facetimed the staff member at the field trip location so that the parent could speak to the child. Per second assistant director(T.D.), the children and field trip staff left the center at around 12:15PM and were headed to Lion Country Safari, in West Palm Beach. The child's mom/stepmom arrived to pickup between 2:30PM and 3:00PM. She paged for him through her walkie talkie. She stated that she did it about 3 or 4 times. Since he did not come to the front office after she paged for him 3 to 4 times, she proceeded to the back to look for him. She checked multiple areas in the facility but didn't find him on-site. She proceeded to call a field trip staff member who located him by grouping all the boys together and calling out his name. She Facetimed the staff member who put him on the phone to speak with the stepmom. She asked him why did he go and he stated that his name was called and he wanted to go. DOCUMENTATION : - The attendance roster was checked for compliance and the child arrived at 10:00AM. The child was signed out of the attendance log at 5:45PM. - The transportation log did not include the child's name. FINIDNGS: Inadequate supervision occurred in this instance, since the child in question was taken on a field trip in the place of another child with a similar name. The child that should have been on the field trip was absent. The school staff didn't notice this error until the parent arrived to pick up the child between 2:30PM and 3:00PM

DOCUMENTATION : - The enrollment form was checked and based on the enrollment form, field trips are not included in the program the summer camp provides and must be purchased separately. - Release of liability form reviewed and based on the form, the child had "permission to be transported by All Sports on filed trips and/or after school pickup". FINDING: Complaint substantiated for taking a child on a field trip that the parents did provide permission for. According to the assistant director, If a parent does not pay or chooses not to pay, the child does not attend a field trip. In this case, the parents did not pay for this field trip, therefore, the child was not granted permission for the child to attend this field trip

The protective covering along the side of the seat (first row) needs to be properly secured. The covering was previously missing and was replaced, but needs to be secured. The deadline to come into compliance has been extended. The other items, cited for being out of compliance during the last inspection, were found to be in compliance during today's revisit inspection

04-01 · Supervision
On several different occasions during this inspections, children were observed unsupervised. Children were observed walking from one area of the facility through the cafeteria to other areas within the facility (spring floor through cafeteria to bathroom, green room through cafeteria to bathroom, office through cafeteria to cubby area, etc.) and no staff members were present in the cafeteria on many of those occasions. Additionally, one child locked himself in a storage/office area and there were no staff members present in the storage/office area. The child did not unlock the door when asked to do so by staff members; one of them went to the office to get the key to unlock the door. Even after the door was unlocked, however, the child would not allow the staff members to open it. A third staff member was called and came over and spoke to the child through the door. The child eventually opened the door and came out of the storage/office area
