During a complaint investigation related to ratio it was noted that the facility was out of ratio when the group of children had varying ages early in the morning. During todays inspection a review of the early morning attendance sheet showed that at 7:37 am this morning there was a total of 12 children of mixed ages (0-4) with two teachers. Two additional staff persons arrived at 7:49. Please submit copies of all mixed group attendance sheets by next Friday June 20, 2025

During a complaint investigation in April a number of areas not in repair were noted on the report. The following items were repaired; the outdoor water fountain was observed in working order today. The broken tiles in the toddler room have been replaced. The loose and broken chairs have been replaced the door frame in the 3 year old classroom has been cleaned. During today's inspection the following repairs were noted: front desk counter border peeling causing rough edges (this was noted during the complaint investigation and has not been corrected) all thresholds leading to outdoor play space were observed to be dirty there are floor tiles in the toddler and preschool classrooms that are starting to crack and should watched climbing structure on outdoor play space was dirty inside the structure (on the floor was mulch and dirt and the structure itself was dirty

Complaint alleges that there are only two teachers that open the school in the mornings, one in the infant room and one in the toddler room and rooms are out of ratio. At times, the toddler teacher has had about 22 children under her care. This morning's face to name attendance was checked for compliance and the following was observed; - Two teachers were at the school from 6:30AM to 7:45AM. There were a total of 15 children of mixed ages (1 to 9) with the two teachers by 7:36AM, prior to the arrival of the third staff member. This complaint is substantiated. Today's ratios were as follows: Infants 2:8, 1y/o 1:6, 1y/o and 2y/o 1:11, 2y/o and 3y/o 1:10, 4y/o and 5y/o 1:11 Ratio in the 2y/o room was out of compliance; there were ten 2y/o children and one 1y/o child with one teacher. TA was provided that minimum ratios must be followed at all times. Director stated that a teacher called out today and they are in the process of hiring another staff member

03-08 · Technical assistance was given during the Routine #2 inspection regarding the two staff in the infant room completed the Infant Toddler Appropriate Practices module from Part II of the DCF child care training. At the time of this inspection there was no documentation to show that one of the staff in the infant room had completed the infant/toddler training. Facility is reminded that this training can be completed for in-service hours as the staff person has already me the requirements for the DCF 40 hour training

At the time of this inspection it was noted that there were not materials in housekeeping, blocks, and , table toys in these classrooms. It was also noted that the amount of materials in the art area and in other areas were not sufficient for the number of children using the classroom. According to staff materials have been added to these room but the children are not taking care the items. so broken items are being discarded. It is suggested that plastic storage bins with lids be added to these classroom to store materials and teachers supervisor play so that materials are used appropriately
