During the time of inspection, the Licensing Specialist reviewed accident/incident reports for L.F. and observed that it did not include parent/guardian signature the day incident occurred. The Provider was unable to obtain parent/guardian signature because Child #1 was withdrawn from the program. CCF Handbook Section 6.4, C Accident/Incident Notification C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program childcare personnel and the custodial parent or legal guardian. 13-03 · Toxic Substances, Hazardous Materials and Weapons
During the time of inspection, the Licensing Specialist observed cleaning supplies on the top of the toilet tank in the bathroom of the one- year-old classroom. The Provider was able to quickly remove and place out of reach of children. This standard was brought back into compliance. Per Section 3.2 Toxic/Hazardous Material/Firearms/Weapons B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times

23-12 · Fire Drills
During the fire drill a teacher failed to possess a current attendance record for the children in care. This standard was fixed at the time of inspection by the Assistant Director gong back inside the facility and grabbing the teacher's iPad that has the class attendance for the day. According to the DCF Facility Handbook: 3.8.4 Fire Drills A. During the facilitys license year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. B. When the facilitys approved fire alarm alert system is activated or initiated, all adults and children must evacuate the facility. C. A current attendance record and parent contact information must accompany childcare personnel out of the building during a drill or actual evacuation and be used to account for all children. The operator must maintain a written record of the fire drills showing the date, number of children and childcare personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1. One fire drill using an alternate evacuation route, 2. One fire drill during napping/sleeping times, and 3. One fire drill in the presence and at the request of the licensing authority. This drill will be coordinated with the operator or designee

28-02 · Bottles, Breastmilk, Formula and Infant Food
Licensing supervisor observed that bottles and/or sippy cups brought from home were not labeled with the child's first and last name. (#10 total throughout the facility) According to the DCF Facility Handbook: 3.9.5 Food Handling A. Bottles and sippy cups provided by the facility must be washed, rinsed, and sanitized between each use and do not have to be labeled. B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily. C. Milk and food must not sit out for longer than 15 minutes prior to the beginning of the meal to avoid contamination and spoilage. Food must not be served to children at more than 110 degrees Fahrenheit. Allow time for food to cool before serving to children that does not exceed 15 minutes. D. Childcare personnel, while distributing snacks and serving food, must use disposable gloves, utensils, or similar items to prevent skin contact with food. E. Food provided by parents/guardians must be properly stored and handled in a sanitary manner at all times to prevent contamination or spoilage. If food is supposed to be kept cold, the food must be stored in a refrigerator until eaten, or parents/guardians must include ice packs to keep food cold

33-06 · Training Requirements
Licensing Supervisor observed one teacher not having shown completion of the Department approved five- hour early literacy course for childcare personnel within 12 months of employment. According to the DCF Facility Handbook: 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all childcare personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the childcare industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Childcare personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C. One college level early literacy course (for credit or non-credit) if taken within the last five years
