Both driver's had expired driver physicals. One vehicle's (chevy) documented inspection from the mechanic was observed expired on 10/3/2025

The following are in need of repair: Peeling paint in 2 year old classroom needs to be repainted. The deep freezer in the kitchen was observed with rust. The refrigerator in the kitchen was observed with rust

A complaint allegation was received on 3/12/2024 alleging that parent witnessed her son being pushed and dragged by the arm. A complaint inspection was conducted on 3/13/2024 and statements received from the Director (LE) and District Manager (IA). Video footage of incident was also reviewed. Video footage review: Complaint Investigator observed the teacher putting out the cots and covering them with fitted sheets. Then, the child moved to an area by the cots and in a blind spot where the camera does not capture his entire body. The teacher was observed walking up to the child, picking him up by one arm and moving him to another area where the other children sat waiting for their cots to be ready for naptime. Per the Director's statement, teacher (LC), only attempted to move the child and he dropped his body. The teacher held him up so that he wouldn't drop to the floor as she moved him form one spot to another. Teacher was placed on administrative leave pending the investigation. Per District Manager's statement (IA), the teacher is on administrative leave pending the investigation. She will return to work and be given further training on handling of a child. This is the first write-up this teacher receives and there have been no previous incidents involving this staff member. This complaint is being closed as Substantiated for pulling the child up by one arm and a class 2 non-compliance is being issued
