At the time of inspection the provider was unable to provide the FSC with a copy of the transportation log. 2.4.2 Transportation Log A log must be maintained for all children being transported in the vehicle or on foot away from and/or to the premises of the home. The log must be retained for a minimum of 12 months. The log must include each childs name, date, time of departure, time of arrival and the signature of the driver verifying all children were accounted for during the visual sweep. A. Prior to transporting children, the transportation log must be recorded, and dated immediately, verifying that all children were accounted for and that the log is complete. B. Upon arrival at the destination, the driver of the vehicle must: 1. Mark each child off the log as the child departs the vehicle; 2. Conduct a physical inspection and visual sweep of the vehicle to ensure that no child is left in the vehicle; and 3. Record, sign, and date the transportation log immediately, verifying that all children were accounted for, and that the visual sweep was conducted. 4. If the provider contracts/agrees with an outside entity/person to provide transportation, then the provider must perform the duties of the driver outlined above in numbers 1-3 The provider adjusted hour of operation to not be licensed during the times her grandchildren are picked up and dropped off for school. Per provider transportation will not be done during her hours of operation

When requested the provider was unable to provide any of the required documents for transporting children. 2.4.4 Vehicle Requirements For the purpose of this section, vehicles refer to those owned, operated or regularly used by the child care home, and vehicles that provide transportation through a contract or agreement with an outside entity. Parents/guardians personal vehicles used for transporting during field trips are excluded from meeting the requirements of this sub-section. G. Emergency medical consent forms or copies of the consent forms signed by the custodial parent or legal guardian and emergency contact numbers must be in the vehicle during transport of children, including field trips. Per provider transportation will not be done during her hours of operation

All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order.
