23-07 · Fire Drills
At the time of inspection, the LS reviewed the facilities fire drill log, which did not display the route taken. the number of staff involved in the drill, and the amount of time it took to complete the evacuation of the facility. The fire drill was also missing documentation of a fire drill being conducted in March of 2025. 3.8.4 Fire Drills A. During the facilitys licensure year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. B. When the facilitys approved fire alarm system is activated or initiated, all adults and children must evacuate the facility. C. A current attendance record and parent contact information must accompany child care personnel out of the building during a drill or actual evacuation and be used to account for all children. When the facilitys approved alarm system is activated, all adults and children must evacuate the facility. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1. One fire drill using an alternate evacuation route, and 2. One drill in the presence of and at the request of the licensing authority in coordination with the operator or designee. A fire drill was conducted at the time of inspection to bring the provider into compliance. A sample fire drill log was provided to the provider containing all elements required in the fire drill log. Although any format of the fire drill log may be utilized, all required components must be met

At the time of inspection five (5) staff files were missing updated Abuse and Neglect Reporting Form. 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. D. Copies of training information and credentials as applicable. E. For the driver only: Florida drivers license and driver physical examination documentation. A copy of the drivers license and the physician certification, or another form containing the same elements of the physician certification, granting medical approval to operate the vehicle, and valid certificate(s) of course completion for first aid training, pediatric cardiopulmonary resuscitation (CPR) procedures, and background screening must also be maintained in the drivers personnel file. F. Prior to beginning volunteering in a child care facility, a CF-FSP 5217, Volunteer Acknowledgment which is incorporated by reference in 65C-22.001(7)(e), F.A.C., and may be obtained from the Departments website www.myflfamilies.com/childcare, must be completed and on file at the child care facility for the volunteer. Written documentation of volunteer hours must be maintained at the facility for a minimum of 12 months and available for review by the licensing authority. The LS suggested annually, perhaps during a staff meeting or at the being of the year when all staff is present having everyone sign annual documents. This may assist in maintaining updated forms. The corrective Action date is April 18th, 2025; to have current staff update the Abuse and Neglect Reporting form
