07-05 · Vehicle Requirements
At the time of inspection, the FSC reviewed the vehicle's alarm system. When reviewing the alarm systems functions the FSC asked the provider to activate the alarm. After removing the key from the ignition, the provider stated she would return to the vehicle and proceeded into the facility. When she returned to the vehicle, she began to attach a plug that was located under the staring wheel. After reinstalling the plug, she then attempted to activate the alarm system by turning on then off the vehicle's ignition. The alarm did not activate. The provider searched the key ring, then stated she would be back. after entering the building and returning to the vehicle. The provider discovered a set of lock keys in the vehicle's dash/cup holder (located under the radio.) At that time the provider began to attempt to activate the alarm by turning on and off the ignition. After several attempts of turning the lock on the alarm system, turning on and off the alarm system, the alarm was successfully activated. 2.5.4 Vehicle Requirements For the purpose of this section, vehicles refer to those owned, operated or regularly used by the child care program, and vehicles that provide transportation through a contract or agreement with an outside entity. Parents/guardians personal vehicles used for transporting during field trips are excluded from meeting the requirements of this sub-section. F. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2. List of approved alarm manufacturers may be found on the Departments website at www.myflfamilies.com/childcare. 3. Alarms must be installed and maintained according to the manufacturers recommendations. 4. Alarm systems installed according to the manufacturers instructions prior to October 1, 2021, will be deemed to satisfy this requirement if they meet the criteria set forth in 2.5.4.F.1. above. Under this allowance, the existing alarm system must always be properly maintained in working order. Should at any time the alarm system require replacement, the new system must be selected from the Departments approved list. Per the provider the system was deactivated to a malfunction with the speaker/voice box. The provider must have the alarm system serviced/ inspected from the Department's approved list to ensure proper operations. At the time of inspection, the Family Service Counselor was unable to locate the facility's license to operate. 402.3125 Display and appearance of license; posting of violations; information to be provided to parents. (1)(a) Upon receipt of a license issued under s. 402.308 or s. 402.309, the child care facility shall display such license in a conspicuous place within the facility. 05-06 · Transportation
At the time of inspection, the FSC reviewed the facility's transportation log. When reviewing the transportation log dated from March 11th to March 15th,2024, the transportation log was not signed by a second staff to confirm that no child was left in the vehicle. 2.5.2 Transportation Log F. Upon arrival at the destination by vehicle, the driver of the vehicle must: 1. Mark each child off the log as the child departs the vehicle; 2. Conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehicle; and 3. Record, sign, and date the transportation log immediately, verifying that all children were accounted for, and that the visual sweep was conducted. 4. Ensure that a second child care personnel conducts a second visual sweep, signs and dates the transportation log verifying that all children were accounted for, and that the log is complete. 5. If the provider contracts with an outside entity to provide transportation, then the provider must assign a child care personnel to perform the duties of the driver outlined above in numbers 1-3. G. Upon arrival at the destination by vehicle, a second and different child care personnel must: 1. Conduct a physical inspection and visual sweep of the vehicle to ensure that no child is left in the vehicle; and 2. Sign, date and record the transportation log immediately, verifying that all children were accounted for, and that the log is complete. The FSC will review transportation logs from March 18th until the reinspection is conducted

17-01 · Outdoor Play Area/Fencing
At the time of inspection, the FSC observed worn and unlevel areas in the ground surfacing of the play area. The FSC also observed wooden logs as barriers on the play area that were very worn. These wooden barriers create a safety hazard as the broken sharp ends cause a threat to the children in care. 3.5 Outdoor Play Area A. The outdoor play area must be clean and free from litter, nails, glass and other hazards. At the time of inspection, the providers contractor came onsite and leveled the uneven surfacing, as well as removed the rooted wooden barriers on the play area. This removed the safety hazards from the play area

At the time of the inspection the FSC reviewed three student files. When reviewing the student files, the FSC observed a file that was missing the student physical health form, the student was enrolled in for more than 30 days. The child was present in care at the time of inspection. 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. At the time of inspection, the FSC reviewed the facilities daily sign in which was inaccurately completed. 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. B. The custodial parent or guardian may document the time when his/her child enters and departs the child care facility or program. However, child care facility personnel are responsible for ensuring that attendance records are complete and accurate. Counselor reviewed transportation logs for 3 months and observed that there were several days with no signature documented for the second childcare personnel. Provider will submit completed transportation logs for the week of 5/15/23-5/19/23 with signature of driver and 2nd childcare personnel. Counselor provided the following technical assistance: Transportation By Vehicle E. 06-02 · Driver's License, Physician Certification
Counselor observed that the driver's physical examination for personnel listed on the supplemental has an invalid date of 12/31/23. Personnel will obtain a current driver physical examination and submit to counselor. Personnel will not operate the vehicle until the physical examination has been completed. Counselor provided the following technical assistance: Records must be maintained and kept current on all childcare personnel, as defined by Section 402.302(3), F.S.
