Staff are required to be fingerprinted every five years. Send the fingerprinting receipts for three staff members.

On February 10, 2022, OCCL received a complaint alleging undocumented accident injuries, lack of supervision, and feeding infants cereal in bottles. An investigation was conducted on February 11, and February 14, 2022. There is no evidence to substantiate that an injury report was not provided because the center had a copy of the report on file and it was documented that Parent #1 refused to sign. This complaint is substantiated for lack of supervision because a child was observed licking juice off of the table without staff intervention, a child fed her snack to three other children without staff intervention, and a staff was not paying attention when a child attempted to bite a child s arm. The complaint is also substantiated for feeding infants cereal in bottles without a note from the child s health care provider.

A complaint was received on August 25, 2021, regarding the allegation of lack of supervision. An unannounced visit was conducted on September 7, 2021. The complaint is substantiated because Staff #1 stated Child #1 did climb over the door and leave the classroom without supervision.

On February 8, 2022, OCCL received a complaint alleging improper staff-to-child ratio. An investigation was conducted on February 11, 2022, and February 14, 2022. This complaint is substantiated for improper staff-to-child because two classrooms were observed to be out of ratio and another classroom was documented as being over ratio on two of the four days of attendance records provided. Additional concerns noted were the following: one owner and a staff member moved children from one classroom to another in an attempt to conceal exceeding maximum group size and told the licensing specialists they did not see what they saw; the other owner was not truthful about the ages of the children in a classroom; Staff #10 provided altered attendance records; attendance records were not made available when requested for other classrooms; Staff #10 attempted to deny entry to the center on February 14, 2022; Staff #8 was working with children without being fingerprinted; the administrator was not at either location on Monday, February 14, 2022, and the owners were not aware of her schedule though she is responsible for both centers, and a staff member at the other center said she typically is not physically present; children s attendance records in the three classrooms were not accurate because there were more children present than signed in; maximum group size was exceeded by nine children in the three to five-year-old classroom; a staff member failed to comply with the Governor s Executive Order by failing to wear a mask; an enrolled infant was removed from a classroom that was out of proper staff-to-child ratio and carried around by the owner, who is not qualified by DEEDS Early Learning to care for children; and one staff member worked for almost nine hours without a break according to attendance records, which does not comply with DE s labor law.

Infant cereal was placed in the bottles by the parents. Discussed with staff to inform parents of OCCL's regulations. Regulation reviewed at the visit. Provide a doctor's note for Child #21124. Child #13 needs an updated infant feeding schedule that is signed by the parent monthly. Send to OCCL when complete.
In the Buzzy Bees 1, petroleum jelly was found on the diaper changing table. No MAR present. This was removed from the classroom. All medication must be stored out of children's reach and have proper documentation.
All staff files must be complete. Staff #1 needs service letter or documentation that two attempts were made.
A licensee shall maintain the staff-to-child ratio at all times. Licensing Specialist Romanelli observed 21 children in the Three's and Four's Classroom. Ms. Jackson moved 1 child to the Three's classroom. Ms. Aja, and Ms. Millie should take Better Kid Care's Online Course; Balancing Child-Staff Ratios. Ensure infants have signed monthly infant feeding schedules for infant classroom.
