On April 22, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, improper staff-to-child ratios, improper mixing of age groups, staff files not being on-site, an enrolled child not having a file on-site, and not having an administrator functioning in that role. The complaint is substantiated for lack of supervision based on interviews and video footage. Child #1 was observed on video to have been on the playground unsupervised for 25 minutes without staff knowledge. Staff #1 was also observed on video leaving children unsupervised in the center on multiple occasions. Staff #1 was terminated from employment as of April 22, 2024. This complaint is substantiated for improper staff-to-child ratio and improper mixing of age groups because based on attendance records received, there were multiple instances of improper staff-to-child ratios and improper mixing of age groups. This complaint is substantiated for staff files not being on-site because when asked to provide staff files, there were no staff files on-site to produce. This complaint is substantiated for an enrolled child not having a file on-site because when asked to provide Child #1 s child file, the required child file elements could not be provided. There is no evidence to substantiate not having an administrator functioning in that role because based on staff interviews, the administrator was functioning in that role. An additional concern noted was attendance records provided were not complete with children not being signed out, not documenting combining classrooms, and staff not signing out of classrooms.
