On October 9, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding caring for children in unlicensed space, lack of security to prevent access to children, staff using cell phones while caring for children, lack of supervision, improper mixing of ages, lack of appropriate materials, and failing to notify OCCL within one business day of an event causing damage that affects the center s ability to operate safely. An investigation began on October 10, 2025, and concluded on October 23, 2025. Based on interviews with the administrator and staff, this complaint is substantiated for caring for children in unlicensed space and failure to notify OCCL within one business day of building damage. Based on interviews conducted, documents reviewed, and observations there is no evidence to substantiate this complaint for staff using cell phones while caring for children, improper mixing of ages, lack of appropriate materials, or lack of security to prevent access to children.
