On April 9, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of lack of supervision. This incident was self-reported to OCCL by Administrator #1 on March 31, 2026. Based on information received in the self-report, this complaint is substantiated for lack of supervision. The administrators acknowledged Child #1 was left behind when the group returned from an outing to a park due to an inaccurate counting of children conducted before leaving the park.

On March 7, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding the allegation of lack of supervision. An unannounced visit was conducted on March 7, 2023, to investigate the complaint. The complaint is substantiated because the center self-reported the incident as required by the Delacare Regulations.

On August 20, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, incorrect staff-to-child ratio, and excessive screentime. An investigation was conducted on August 21, 2024. This complaint is substantiated for incorrect staff-to-child ratio because according to staff interviews and documentation received, multiple classrooms were not in correct staff-to-child ratio on August 20, 2024. One classroom had incorrect staff-to-child ratios for an hour and 27 minutes. There is no evidence to substantiate this complaint for lack of supervision because according to staff interviews, all staff denied children being left alone in the hallway. There is no evidence to substantiate this complaint for excessive screentime because according to staff interviews, all staff denied using excessive screentime in the classrooms. An additional concern noted at the visit was staff attendance records on the documentation received were inaccurate as three staff did not sign out of their classrooms and there was one instance of no staff member being signed into a classroom for one hour.

On February 13, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of not providing sufficient milk to the children, not providing water for the children, cockroaches in the center, gnats around sinks, improper staff-to-child ratios and improper mixing of toddlers and preschool age children, and school-age children walking unsupervised. An investigation was conducted on February 21, 2024. This complaint is substantiated for improper staff-to-child ratios because a classroom was out of staff-to-child ratio on two days based on a review of attendance records. There is no evidence to substantiate this complaint for improper mixing of toddlers and preschool-age children, as the center is combining them during the timeframes permitted. There is no evidence to substantiate this complaint for not providing sufficient milk because 12 gallons of milk were present at the visit and all staff interviewed stated milk is served to children at meals. There is no evidence to substantiate this complaint for not providing water for the children based on staff interviews conducted. All staff members interviewed stated children are offered water throughout the day and provided some if they ask. There is no evidence to substantiate this complaint for the presence of cockroaches or gnats in the center based on observation of the kitchen, classrooms, and sinks in the facility. A previously conducted complaint investigation showed the center is completing regular pest control treatments specifically for cockroaches. There is no evidence to substantiate this complaint for school-age children walking unsupervised based on interviews conducted. The facility director stated there are no children enrolled from P.S. duPont Middle School, and any school-age children enrolled at the center are transported to and from their schools. An additional concern noted was staff and child attendance records were incomplete due to children and staff not signing in or o ut as needed.

On February 7, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of a cockroach infestation. This complaint is substantiated because the center provided documentation of a recent contract with Home Paramount Pest Control to conduct weekly visits for cockroaches, showing there was a recent infestation.

On July 26, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of staff not being qualified, lack of supervision, menus not being posted, and the administrator not being present at the facility the required number of hours. An unannounced visit was conducted on August 1, 2023, to investigate this complaint. This complaint is substantiated because the attendance records from June 29, 2023, showed Staff #2 alone with 22 school-age children for the entire day. The attendance records from July 28, 2023, documented Staff #3 present in the summer camp classroom. Staff #3 had not been identified to OCCL as a staff member and had not been fingerprinted prior to their start date. There was not a current menu posted in any of the classrooms or the kitchen. Mr. Purnell was not able to provide a copy of the current menu either. During the complaint visit, there were other concerns cited. This included Staff #1 stating she did not have any attendance records for OCCL to review and that she did not have access to ProCare so that she could document the attendance, and the attendance records that were reviewed for the summer camp program did not have any of the children or staff members signed out on any of the dates that were reviewed.

On March 1, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding the allegations the staff is not following the center s food policy concerning procedures that address food allergies and the staff did not notify the child s guardian of the allergic reaction as required. An unannounced visit was conducted on March 7, 2023, to investigate the complaint. The complaint is substantiated because through interviews it was determined the child did ingest peanut butter and was placed on the bus to be transported to another school before the guardian or the school had been notified of the incident.

The Office of Child Care Licensing (OCCL) received a complaint on February 8, 2023, regarding allegations of lack of supervision and unqualified staff alone with the children. An unannounced visit was conducted on February 14, 2023, to investigate the complaint. The complaint is substantiated for lack of supervision because Child #1 did leave the classroom and was unsupervised until he was stopped by another staff member and returned to the classroom. Staff stated Child #1 never left the building. There is no evidence to substantiate the allegation of unqualified staff alone with children because the attendance records document both Staff #1 and Staff #2 were signed in to the classroom and Staff #2 had not been left alone with the children.

The Office of Child Care Licensing (OCCL) received a complaint on December 13, 2022, regarding allegations of improper discipline and improper staff-to-child ratios. An unannounced visit was conducted on December 19, 2022. The complaint is substantiated for improper discipline. OCCL cited the non-compliance when the administrator self-reported the incident. Staff #1 s employment was immediately terminated. There is no evidence to substantiate the allegation of improper staff-to-child ratio because the attendance records showed the correct ratios were being maintained.

The Office of Child Care Licensing (OCCL) received a complaint on October 27, 2022, regarding the allegations of improper staff-to-child ratios, staff not being qualified, the administrator not being present as required, and improper discipline. An investigation was conducted on November 1, 2022. The complaint is substantiated for improper staff-to-child ratios and unqualified staff because during the investigation the specialist observed the one-year-old classroom with an unqualified staff member alone with six children. This was corrected at the visit when Administrator Roxanne Coleman entered the classroom. Based on staff interviews and observations, there is no evidence to substantiate the allegations of staff using improper discipline. There is no evidence to substantiate this complaint for the administrator not being present the required number of hours as she stated she is present at the center from 8:30 am to 7:00 pm and on days when she may not be present, there are other staff present to assist when needed. Concerns noted were staff and child attendance were not documented in one classroom.

On June 8, 2026, and June 22, 2026, OCCL received complaints regarding allegations of regulatory non-compliance in the following areas: infant safe sleep, infant feeding requirements, infant daily logs, age-appropriate toys and equipment, required activity areas, unqualified staff alone with children, no notification to OCCL of a change in administrator, inappropriate behavior supports, indoor space requirement violations, monthly fire inspection reports and fire drills not completed, no notification to OCCL of failure of fire alarm system, unsafe transportation, staff background check requirements, lack of building security, unsafe outdoor play area, no staff access to child emergency information, policy for child release, no notification to OCCL of child injury requiring medical care, lack of paper towels for hand washing requirements, staff using disrespectful communication to other staff, staff termination policy, written policies available to staff, staff files current, child present without required documentation, incorrect staff-to-child ratios. An investigation began on June 9, 2026, and concluded on June 30, 2026. Based on observations, staff interviews, and documentation reviewed this complaint is substantiated for failing to complete monthly fire inspection reports and file drills, staff background check requirements, no notification to OCCL of child injury requiring medical care, and an unenrolled child present without required documentation and being cared for in unlicensed space. Based on interviews, observation, and documents reviewed, there is no evidence to substantiate this complaint for safe sleep violations, infant feeding requirements, infant daily log, insufficient age-appropriate toys and equipment, required activity areas, unqualified staff alone with children, no notification to OCCL of a change in administrator, inappropriate behavior supports, indoor space requirement violations, no notification to OCCL of failure of fire alarm system, un safe transportation, lack of building security, unsafe outdoor play area, no staff access to child emergency information, policy for child release, lack of paper towels for hand washing requirements, staff using disrespectful communication to other staff, staff termination policy, written policies available to staff, staff files current, or incorrect staff-to-child ratios.
